Investigation On Cognition,Attitude And Behavior Of Professional Spirit Among Rural Medical Staff

Objective:A self-designed questionnaire on occupational cognition,attitude and behavior of rural medical staff was used to investigate the status quo of occupational cognition and attitude among rural medical staff in Chengde,Hebei province.This paper probes into the elements of professional spirit related to rural medical personnel,especially the principles and responsibilities mentioned in the doctor’s manifesto,to further promote the construction of rural medical personnel professional spirit.Methods:1 The development of questionnaire on rural medical workers’professional spirit cognition,attitude and behavior:firstly,on the basis of consulting a large number of relevant documents,15 rural medical personnel in Chengde City,Hebei Province,were selected to conduct semi-structured interviews.The initial questionnaire was formed,then the questionnaire was revised and perfected through two rounds of expert consultation and pre-investigation,and the reliability and validity of the questionnaire were tested.Finally,the questionnaire of professional spiritual cognition,attitude and behavior of rural medical personnel was formed.2 Analysis of the status quo of rural medical workers’professional spirit and demographic characteristics:a stratified sampling method was used to investigate 759 rural medical personnel in Chengde City,Hebei Province.The independent sample t test and X~2 test with ANOVA analysis were used to understand the present situation of the professional spirit of the rural medical personnel in Chengde City,Hebei Province.Results:1 The questionnaire of vocational mental cognition,attitude and behavior of rural medical staff was prepared and formed.The questionnaire was mainly composed of two parts:the first part was the general questionnaire;The second part is the questionnaire of vocational mental cognition,attitude and behavior of rural medical staff,including professional ability(7 items)and keeping secret for patients(6 items),industry autonomy(7 items),and patients maintain appropriate relationship(4 items),patients’interests first(4 items),informed consent(4 items)six dimensions,a total of 32 items.Questionnaire Cronbach’sɑcoefficient is 0.871,the dimension of Cronbach’s alpha coefficient is 0.703~0.905,retest reliability value of 0.961.The S-CVI of the questionnaire was 0.902,and the average CVI value of each dimension was0.778~1.000.The results of exploratory factor analysis showed that six factors were extracted from the questionnaire,and the cumulative interpretation variation was 53.304%,and the load of each item factor was greater than 0.4.2 Among the rural medical staff,66.5%of the men and 33.5%of the women were males.Only 1.7%of people under 30 years old and 48.1%above50 years old.At present,only 4.2%of college graduates are educated,and most of them still focus on the secondary technical level accounting for 63.7%,while the tertiary level accounts for 24.8%.85.8%of rural medical staff have only rural doctors’certificates,79.7%of rural medical staff work seven days a week,and 48.6%of rural medical staff work more than 12 hours a day.Annual income in 5000 yuan of the following accounts for 15.2%,only 10.4%of the annual income in 35000 yuan of above,part of the medical personnel need to rely on farming or other sideline to increase their income.3 The overall cognition of the professional spirit of the rural medical staff was in the middle level,the total score was 121.00±8.93,and the items were3.78±0.28.In a professional capacity,patients’interests first,keeping secret for patients and industry autonomy dimensions of cognitive level is higher.The average scores of items were more than 3.5,respectively(4.21±0.46)(4.17±0.53)(3.80±0.31)and(3.66±0.37).The cognitive level of informed consent and proper relationship with patients was lower,and the mean scores of items were(3.30±0.36)(3.31±0.74).4 The professional spirit of rural medical staff has statistical significance in age,area,education level,qualification and working time of the week(P<0.05);The level of professional mental cognition in the ages of 31~40 was the highest.Weichang County scored the highest,the scores of rural medical staff with undergraduate degree level were higher than that of other medical staff,the highest score for assistant physician qualifications,rural medical staff who worked six days a week scored higher than other working hours.Conclusion:1 This study prepared a questionnaire for the professional spirit cognition,attitude and behavior of rural medical staff with good reliability and validity,and the results were reliable.2 The gender ratio of rural medical personnel is out of balance seriously,the phenomenon of age aging is obvious,the educational level is low,the degree of professionalization is low,the working hours are long,the income level is on the low side.3 The total Professional spiritual cognition of rural medical personnel was 121.00±8.93,and the items were 3.78±0.28.The total professional spiritual cognition was at the middle level.There were some differences in the professional spirit of rural medical staff with different population characteristics.
