Objective: To explore the value of hand assisted laparoscopic radical gastrectomy for advanced gastric cancer by comparing the results of hand assisted laparoscopic, laparoscopic and open radical resection of the distal gastric carcinoma. Methods: The results of 80 cases of distal radical gastrectomy for gastric cancer during the period from August 2015 to December 2017 were analyzed retrospectively, including 30 cases of hand assisted laparoscopic surgery,25 cases of laparoscopic assisted surgery, and 25 cases of laparotomy. The observation indexes include the operation time, the number of lymph node dissection, the amount of intraoperative bleeding, the length of incision, the time of postoperative exhaust, postoperative complications, the time of hospitalization and the cost of hospitalization.Results:1.There are no significant difference between the three surgical methods in intraoperative lymph nodes, postoperative hospitalization expenses and postoperative complications in the near future.2.In the intraoperative bleeding volume, postoperative exhaust time, hospitalization timeand the length of incision,Hand assisted laparoscopic group was better than the laparotomy group and there was no significant difference at the time of surgery.3.The hand assisted laparoscopic group was better than the laparoscopy group in the operation time. The amount of bleeding, postoperative exhaust time, hospitalization time and the length of incision showed no significant difference.4.The amount of bleeding in operation in laparoscopic group, postoperative exhaust time, hospitalization time and the length of incision is better than the laparotomy group. At the time of surgery was significantly longer than laparotomy group.Conclusion:1.Hand assisted laparoscopic distal gastrectomy can achieve a radical effect of open surgery for tumor resection and lymph node dissection, and does not increase the economic burden of patients.2.Hand assisted laparoscopic surgery has the advantages of less postoperative bleeding, faster postoperative recovery. And the operation time is short, lower than the laparoscopy assisted operation.
