Objective: Use glycine sandblasting and subgingival ultrasonic to remove the plaque in the periodontal pocket,comparison that on the improvement situation of the clinical indicators and change of porphyromonas gingivalis(Pg)content in periodontal main pathogenic bacteria.Choose a optimal therapy for future maintenance therapy of chronic periodontitis through the experimental results.Methods:1.Experimental subjects: From January 2017 to November 2017 in hebei medical university stomatological hospital during periodontal disease in patients with chronic periodontitis,and after a complete periodontal treatment with the same periodontist,it has been maintained for at least one month.2.Grouping: Adopt self-control design principles.Use glycine sandblasting to remove subgingival plaque on the left side and as sandblasting group,use ultrasound to remove subgingival plaque on the right side and as ultrasound group.3.Experimental methods: Before the treatment,left and right side were randomly selected four sites into the experiment site from all sites that the depth of periodontal pocket 3~6mm.The records of clinical indicators:probing the depth,bleed on probing,gingiva groove liquid collection and the percentage of BOP(BOP%)is counted.Two teeth were randomly selected from left and right quadrant,and the plaque index was checked by the inspection method with the combination of visual diagnosis and probe.One week and one month after surgery,recording clinical indicators and collecting gingival groove liquid that consistent with the test site of the baseline selection.The collected gingival groove liquid that using real-time PCR technology todetect the relative content changes of Pg to total bacteria in subgingival plaque at three points.Results:The study included 29 patients,including 14 men and 15 women,one of the women missed a week after the surgery.The mean age of patients was 48.8+8.4.Oral plaque control is good in the whole treatment,the preoperative plaque index of sandblasting group and ultrasonic group was 0.86 and 0.84 respectively.After treatment,both groups were stable at 0.64.Both PD and BOP% decreased significantly one month after surgery,PD was still improved a month after surgery(P < 0.05),BOP% is equal to the level of a week,but there was no significant difference between the two groups at the same time before and after treatment(P>0.05).The content of Pg relative to total bacteria decreased longitudinally,but there was no statistical difference between the groups before and after treatment(P > 0.05).All the subjects showed no adverse reactions to subcutaneous emphysema,and all indicated that sandblasting treatment was more comfortable and more willing to accept.Conclusions:1.During the periodontal maintenance treatment stage of chronic periodontitis patients,clinical indicators showed significant improvement after use the glycine sandblasting to remove subgingival plaque.2.In removing subgingival plaque,the subgingival sandblasting can obtained the same therapeutic effect with subgingival ultrasound.3.The relative content of pathogenic bacteria Pg in periodontal pockets was significantly reduced after the treatment of subgingival glycine sandblasting,which could effectively control periodontal inflammation.4.Glycine sandblasting is easy to operate,safe and efficient,and good comfort is more acceptable to patients.In the long-term periodontal maintenance treatment,it can improve the patient's rediagnosis rate and better maintain the long-term effect of periodontal treatment.
