Study Of Status Of Cardiovascular Disease,Related Risk Factors And Prognosis Of Peritoneal Dialysis Patients

Objective Chronic renal failure(Chronic renal failure,CRF)is the final stage of end-stage renal disease(End-stage renal,disease,ESRD),the mechanism for kidney and renal function in chronic kidney disease(Chronic renal,disease,CKD)are in case of loss,and the irreversibility of promoting poison retention and metabolites,and disrupt the acid-base balance of water and electrolyte,cause endocrine disorders,and ultimately to the development of ESRD.ESRD has high disability and mortality rate,and has huge cost of treatment.As a chronic major disease,it poses a serious threat to human health and brings heavy burden to families and society,so it is very nessary to investigate the status of cardiovascular disease,related risk factors and prognosis of peritoneal dialysis patients.Methods 80 cases of patients with end-stage renal disease who received peritoneal dialysis treatment at least 3 months from May 2015 to May 2017 in our hospital were randomly selected,these patients were divided into associated group(n=40)and not associated group(n=40)two groups on the basis of associated with comorbid cardiovascular disease,using cross-sectional retrospective study method,the blood pressures,heights and weights,FBP,medical history of the two groups were collected,and then the body mass index was calculated,at the same time,the laboratory indexes of the two groups were collected,the follow-up time and prognosis data of patients undergoing peritoneal dialysis treatment from the end of follow-up to death were collected,at the same time,the the death reasons were recorded in detail,then the genders,primary diseases,ages,heights,weights,body mass indexes,blood pressures,blood glucoses,dialysis time,experiment indexes,clinical outcomes,1 years,2 years,3 years survival of the two groups were statistically analyzed,and the cardiovascular disease risk factors of peritoneal dialysis patients,correlation analysis of the causes of death of peritoneal dialysis patients,the survival rates of related factors of peritoneal dialysis patients were analyzed by univariate and multivariate COX analysis.Results The age of the associated group was significantly higher than the non accompanying group(P<0.05),but the differences of genders,heights,weights,body mass indexes,dialysis time and primary diseases between the two groups were not significant(P>0.05).The serum Hb and Alb levels of the associated group were significantly lower(P<0.05),the SCr,CRP,P and TG levels were significantly higher than the non associated group(P<0.05),but the differences of diastolic blood pressures and systolic blood pressures,serum FPG,LDL-C,HDL-C,TC,Ca,PTH,BUN,UA levels between the two groups were not significant(P>0.05).The survival rate of the associated group was 77.5%(31/40)was significantly lower than the non associated group 92.5%(37/40)(P<0.05),and the mortality rate 30.0%(9/40)was significantly higher than the not associated group 7.5%(3/40)(P<0.05).The 1-,2-and 3-year survival rates of the associated group 90.0%(36/40),85.0%(34/40),77.5%(31/40)were significantly lower than the not associated group 97.5%(39/40),95.0%(38/40),85.0%(34/40)(P<0.05).The risk factors associated with cardiovascular disease of peritoneal dialysis patients included SCr,Alb,CRP,and TG(P<0.05).Among the 80 cases of peritoneal dialysis patients,12 cases died.In terms of causes of death,4 cases were cardiovascular disease,3 cases were cerebrovascular diseases,2 cases were severe infection,and 3 cases were others,accounting for 33.3%,25.0%,16.7% and 25.0% respectively.Univariate COX analysis showed that the related factors of survival rate of peritoneal dialysis patients included age,systolic blood pressure,diastolic blood pressure and CVD(P<0.05),excluded BMI,Hb,Alb,CRP,Ca,P,UA,BUN,UA,Qi,Yang,Qi,and Yang(P>0.05).Multivariate COX regression analysis showed that the related factors of survival rate of peritoneal dialysis patients included age,CVD,CRP and HDL-C(P<0.05),excluded TG,systolic blood pressure and diastolic blood pressure(P>0.05).Conclusion Peritoneal dialysis patients have high probability of cardiovascular disease,the risk factors include SCr,Alb,CRP,TG,the main cause of death are CVD,the peritoneal dialysis with cardiovascular patients have high mortality,poor prognosis,the survival rate related factors of peritoneal dialysis patients include age,CVD,CRP,HDL-C.
