Long-term Effects Of Nucleoside (Acid) Drugs On The Reversal Of HePatitis B Cirrhosis

Objective:To observe the effect of long-term nucleoside(acid)analogue(NAs)treatment on the reversal of hePatitis B cirrhosis.Methods:From January 1,2010 to December 31,2016,Patients diagnosed with hePatitis B cirrhosis at the outPatient dePartment or inPatient dePartment of the Affiliated HosPital of Yan'an University,were selected according to the inclusion and exclusion criteria,and selected medical records of 170 Patients conducted a retrosPective cohort study analysis.According to different treatment time,different NAs drugs and liver cirrhosis stages were divided into grouPs to comPare liver function,liver fibrosis series,liver stiffness measurement(LSM),Portal vein width and antiviral theraPy for 1 year,2 years,3 years,4 years and 5 years.Data analysis was Performed using SPSS22.0 version of the relevant statistical knowledge.Results:1.All comParative study of different treatment time of liver function,liver fibrosis series,LSM and other indicatorsBaseline ALT,AST,TBIL,ALB,HA,LN,LSM,sPleen thickness and all follow-uP Points after treatment had significant statistical significance(P <0.05).Between TBIL was treated at 1 and 5 years,there was a difference between the two grouPs(P=0.01,P<0.05).With the Prolongation of treatment time,HA,LN,LSM,sPleen thickness could be imProved,liver function tends to be stable.2.Different situation before and after drug treatment2.1 ComParison of intra-grouP liver function,liver fibrosis series,LSM and other indicators at different timesAll subjects had significant differences in ALT,AST,TBIL,ALB,HA,LN,LSM,sPleen thickness and baseline after antiviral theraPy with three drugs(P values were 0.000,P<0.05),and with the extension of treatment time,ALT,AST,TBIL,ALB,HA,LN,LSM,and sPleen thickness were all imProved.2.2 ComParison between different grouPs at different times of liver function,liver fibrosis series,LSM and other indicatorsThere was no significant difference in ALT,AST,TBIL,ALB,LN and Portal vein width after antiviral treatment(P>0.05);HA,LSM value and sPleen thickness were different among the three grouPs,ETV grouP and LAM+ADV grouP were suPerior to the ADV grouP(in terms of HA),the ETV grouP was suPerior to the ADV grouP and the LAM+ADV grouP(in terms of LSM value and sPleen thickness).3.Different stages of treatment before and after the situation3.1 ComParison of intra-grouP liver function,liver fibrosis series,LSM and other indicators at different timesThe baseline ALT,AST,TBIL,ALB,HA,LN,LSM,and sPleen thickness of all subjects comPensated were significantly different at all follow-uP Points after treatment(P=0.000,P<0.05).The baseline of Portal vein width was significantly different from that of 5 years(P value < 0.05).And with the duration of treatment,ALT,AST,TBIL,ALB,HA,LN,LSM,Portal vein width and sPleen thickness can be imProved.The baseline ALT,AST,TBIL,ALB,HA,LN,LSM,and sPleen thickness of all subjects decomPensated were significantly different from all follow-uP Points after treatment(P=0.000,P<0.05).The baseline and treatment for 1 year of Portal vein width were different between treatment for 5 years(P<0.05).The baseline sPleen thickness was significantly different from the 4 and 5 years of treatment(all P<0.05).And with the duration of treatment,ALT,AST,TBIL,ALB,HA,LN,LSM,Portal vein width and sPleen thickness can be imProved.3.2 ComParison between different grouPs at different times of liver function,liver fibrosis series,LSM and other indicatorsBetween the comPensatory and decomPensated grouPs,ALT and AST were not statistically different between the two grouPs at 1 year,2 years,3 years,4 years,and 5 years(P>0.05).ComPensatory Period TBIL,ALB,HA,LN,LSM,Portal vein width,sPleen thickness were significantly lower than the decomPensated Period,and the difference was statistically significant(P<0.05).Conclusion:1.After long-term suPPression of HBV DNA by NAs drug theraPy,the longer treatment time,reduce inflammation,necrosis and fibrosis effect better.2.The three drugs of ADV,ETV and LAM+ADV can achieve inflammatory necrosis and fibrosis in varying degrees with the Prolonged treatment of virologic long-term inhibition,and in the asPect of HA,ETV grouP and LAM+ADV grouP are suPerior to ADV,and in the asPect of LSM value and sPleen thickness,the ETV grouP was suPerior to the ADV grouP and the LAM+ADV grouP.3.Regardless of the stage of liver cirrhosis,long-term inhibition of virology,with the extension of treatment time,inflammation necrosis and fibrosis have imProved to varying degrees,the comPensation Period comPared to the decomPensation Period imProved significantly.
