Objective:The purpose of this study was to investigate the correlation between serum levels of soluble CD36(sCD36),Resistin and type 2 diabetes mellitus(T2DM)combined with nonalcoholic fatty liver disease(NAFLD).Methods:A total of 200 type 2 diabetic patients were admitted to Department of Endocrinology,Affiliated Hospital of Chengde Medical College from March 2016 to October 2016.Of these,176 met the inclusion criteria.They were divided into non-NAFLD group(n=56),mild NAFLD group(n=45)and moderate and severe NAFLD group(n=75)according to the ultrasound result.Collect 3 sets of general information,just as,glycosylated hemoglobin,fasting blood glucose,fasting C peptide,postprandial 2h C peptide,waist circumference,triacylglycerol(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),alanine transaminase(ALT),gamma-glutamyl transferase(γ-GT),aspartate transaminase(AST)and so on.Body mass index(BMI)was calculated.Serum sCD36 and resistin were measured by ELISA method.Results:1.General information and sCD36,Resistin level comparisonThere was no significant difference in age,height and γ-GT between the three groups(P>0.05).The levels of body mass index,waist circumference,plasma TG,total cholesterol,low density lipoprotein cholesterol,AST,ALT,glycosy-lated hemoglobin,fasting blood glucose,fasting C peptide and postprandial 2 h C peptide,serum sCD36[(4.34±0.21)、(7.25±0.23)μg/L],Resistin [(2.59±0.36)、(6.56±0.56)]μg/L] in mild NAFLD group,moderate and severe NAFLD group were higher than those non-combined NAFLD group,the difference was statistically significant(P<0.05),and high density lipoprotein cholesterol was lower than that of uncombined NAFLD group(P<0.05);The levels of body mass index,waist circumference,serum TG,TC,LDL-C,ALT,AST,HbAlc,fasting blood glucose,fasting C-peptide,postprandial 2h peptide,sCD36 and Resistin in patients with moderate and severe NAFLD were higher than those in mild NAFLD Group(P<0.05),HDL-C levels were lower than those in mild NAFLD group,the difference was statistically significant(P <0.05).2.Correlation Analysis of Serum sCD36,Resistin Level and Biochemical IndexesPearson correlation analysis showed that serum sCD36 and Resistin levels were positively correlated with body mass index,waist circumference,plasma TG,ALT,fasting blood glucose(P<0.05),and serum sCD36 levels were positively correlated with Resistin(r=0.388,P <0.05).3.Logistic Regression Analysis of sCD36,Resistin and Nonalcoholic Fatty LiverLogistic regression analysis showed that serum sCD36(OR=5.770,95%CI:2.789~11.941,P=0.001),Resistin(OR=2.791,95%CI: 1.565~4.976,P=0.001)were risk factors for T2 DM with NAFLD,and it will increase the risk.Serum sCD36(OR=5.72,95% CI:1.201-2.290,P = 0.001)and Resistin(OR=1.89,95% CI: 0.345-0.934,P = 0.001)were positively correlated with the severity of NAFLD.Conclusion:1.Patients with T2 DM and NAFLD had more obvious disturbance of glucose and lipid metabolism,and increased with the increased severity of NAFLD.2.sCD36 in patients with T2 DM complicated by NAFLD had higher levels of Resistin and increased with the increased severity of NAFLD.3.Serum sCD36 and Resistin are risk factors for NAFLD in T2 DM patients,and it will increase the risk,which is positively correlated with the severity of fatty liver.
