Objective: To investigate the role of endoscopic ultrasonography(EUS)combined with endoscopic resection in the diagnosis and treatment of gastric stromal tumors.Methods:A retrospective analysis was performed on 72 patients with pathologically confirmed gastric stromal tumors who underwent endoscopic resection after undergoing EUS examination from May 2012 to February 2018 in the Department of Gastroenterology,Second Hospital of Hebei Medical University.The features of preoperative EUS examination of lesions and ultrasound diagnosis were recorded;endoscopic resection,surgical procedures,complications,and complete extent of lesion resection were recorded,also.To evaluate the accuracy of EUS in the diagnosis of gastric stromal tumors,to compare the characteristics of EUS images with different risk grades,and to analyze the extent and complications of EUS-assisted resection of gastric stromal tumors by different endoscopic resection methods.Results: The prevalence of preoperative EUS and postoperative pathology in the diagnosis of gastric stromal tumors was 76.4%(55/72).Non-homogeneous internal echo,liquid non-echo zone,and calcification often indicate a higher risk of gastric stromal tumors(P<0.05);There was no significant correlation between tumor location,echo pattern,internal blood flow signals,and tumor surface characteristics with the risk of gastric stromal tumors(P>0.05);The complete resection rate of gastric stromal tumors under endoscopy was 95.8%(69/72),Intraoperative bleeding occurred in 2 cases(2.8%)and perforation in 13 cases(18.0%),all cases were successfully treated by endoscopy.Postoperative electrocoagulation syndrome occurred in 7 patients(9.7%),all recovered after conservative treatment,Postoperative perforation occured in 1 case,recovered after surgical treatment.Conclusion:Endoscopic ultrasonography is of great significance for the diagnosis and evaluation of the risk of gastric stromal tumors,and it can be used to assist endoscopic resection of gastric stromal tumors.Endoscopic resection of gastric stromal tumors has great feasibility and safety.
