Objective:To investigate the expression of serum high mobility group box 1(HMGB1)in patients with systemic lupus erythematosus(SLE)and its clinical significance.Methods:Colecting 60 patients diagnosed with SLE and 20 healthy volunteers in Department of Rheumatology and Immunology of Tianjin Medical University General Hospital,the clinical symptoms and related serological indicators of each patient were recorded.SLE patients were grouped as follows:According to the SLEDAI scores,patients were divided into two groups:active group(SLEDAI?10,n=29)and inactive group(SLEDAI<10,n=31);According to LN diagnostic criteria,patients were divided into LN group(n=36)and non-LN group(n=24);According to the clinical symptoms accompanying the patients,they were divided into fever group(n=29)and no fever group(n=31),photosensitivity group(n=30)and no photosensitivity group(n=30),arthritis group(n=21)and non-arthritis group(n=39),Raynaud's phenomenon group(n=7)and no Raynaud's phenomenon group(n=53),serositis group(n=29)and no serositis group(n=31),anemia group(n=26)and no anemia group(n=34),leukopenia group(n=25)and no leukopenia group(n=35),thrombocytopenia group(n=17)and no thrombocytopenia group(n=43).Serum levels of HMGB1 in each group were detected by ELISA,and the correlation between HMGB1 and clinical indexes was analyzed by linear correlation analysis respectively.Results:1.Serum HMGB1 levels were elevated in healthy controls,inactive SLE patients,and active SLE patients(P<0.01).Compared with inactive group,active group had higher SLEDAI,anti-ds-DNA antibody and lower C3,C4(P<0.05).2.The level of serum HMGB1 and 24h urinary protein(24hUpro)in LN group were significantly higher than those in non-LN group(P<0.01).3.The level of serum HMGB1 was similar between different clinical symptoms groups,the diference was not statistical y significant(P>0.05).4.The level of serum HMGB1 in SLE patients was positively correlated with SLEDAI(r=0.505),anti-ds-DNA antibody(r_s=0.424),anti-C1q antibody(r_s=0.288),24hUpro(r_s=0.333),but was negtively correlated with C3(r=-0.302)(P<0.05).Conclusion:1.The level of serum HMGB1 in SLE group was higher than that in healthy control group.2.The level of serum HMGB1 in active group was higher than that in inactive group and was positively correlated with SLEDAI,anti-ds-DNA antibody,but was negtively correlated with C3.This may prompt that elevated serum HMGB1 may indicate SLE disease activity.3.The level of serum HMGB1 in LN group was higher than that in non-LN group and was positively correlated with 24hUpro and anti-C1q antibody,which reveals that elevated serum HMGB1 levels may be involved in the development of renal impairment in SLE patients.
