| Objective:Using fMRI to study the central mechanism of primary dysmenorrhea with puncturing sanyinjiao,aiming at providing visual evidence for acupuncture clinical application.Methods:16 PD subjects were randomly divided into sanyinjiao group(group A)and non-acupoint group(group B),each group included 8 subjects.All subjects were observed three periods in the baseline time,and were matched the Visual Analogue Scale which score more than 4cm.The acupuncture treatment were arranged in the first day to the third day of the subjects period.And there would be two fMRI scanning and VAS scoring before and after the acupuncture treatment.Finally calculated the ReHo value according to the image before and after acupuncture of the two groups.And analyzed the differences of brain function in these two groups before and after acupuncture.At the same time,analyzed the pain degree by VAS scores in these two groups by using SPSS22.0,concluded the immediate effects of acupuncture.Results:1.Subjects brain’s function changes in Group A: compared with pre-acupuncture,brain areas appeared increasing ReHo value after acupuncture as follows: the medial prefrontal cortex,the right side of orbitofrontal cortex,the left side of the dorsomedial prefrontal cortex,right anterior cingulate gyrus,right l gyri temporalis superior,ect;decreasing ReHo value brain areas as follows: right calcarine sulcus,putamen and so on.2.Subjects brain’s function changes in Group B: compared with pre-acupuncture,brain areas appeared increasing ReHo value after acupuncture as follows: the left middle temporal gyrus,the left postcentral gyrus,left ceneus,ect.;decreasing ReHo value brain areas as follows: the left precentral,right medial prefrontal cortex,right gyri temporalis superior and so on.3.Subjects’ brain areas ReHo value changes between those two groups: the ReHo value increasing area as follows: the left dorsolateral prefrontal cortex,left medial prefrontal cortex,right anterior cingulate;Re Ho value decreased in the right thalamus,left lingual gyrus.4.The VAS scores of two groups before and after acupuncture: VAS scores had statistical significance in group A before and after acupuncture,indicating that dysmenorrhea had relieved and the pain degree decreased;VAS scores had no statistical significance in group B before and after acupuncture,indicating that dysmenorrhea had not relieved.Comprared within the two groups after acupuncture,VAS scores had statistical significance,which indicated the difference in curative effect in these two groups,group A showed better than group B in relieving dysmenorrhea.Conclusions:1.The Re Ho value changes in the cingulate and prefrontal cortex and thalamus may be involved in the central mechanisms of acupuncture sanyinjiao to treat dysemenorrhea.2.The ReHo value changes in the temporal lobe and the postcentral gyrus may be involved in the central mechanisms of acupuncture non-acupoint to treat dysemenorrhea.3.The putamen and ceneus may be involved in the analgesic effect of acupuncture on PD,and the relation between the calcarine sulcus and the lingual gyrus may be related to the evaluation of pain.4.Acupuncture sanyinjiao can relieve dysmenorrhea symptoms and reduce VAS scores. |
