Objective To provide reference for clinical safe and effective use of anti-emetic drugs and promote the rational use of anti-emetic drugs,we evaluate the rationality of anti-emetic drugs prescribing and analyze the causes of irrational utilization by investigating utilization of anti-emetic drugs in one hospital in 2017.Methods According to the inclusion criteria,patients with malignant tumor undergoing chemotherapy in our hospital in 2017 were selected as the study subjects for retrospective investigation.The patient's general condition,the basic condition of the disease,the application of chemotherapy drugs and utilization of anti-emetic drugs from the medical record were recorded in Excel spreadsheets,and then we established a database in the light of the unified standard.Consulting the ?NCCN Anti-emetic Guidelines 2017 V2?,?ESMO Anti-emetic Guidelines 2016?,?Tumor Treatment-Related Vomiting Prevention Guidelines2014 ? and related drug instructions,we construct prescription evaluation standard of utilization of anti-emetic drugs,and analyze the rationality of utilization of anti-emetic drugs from all included data.Results The results showed that a wide range of irrational utilization of the chemotherapy anti-emetics drugs was found among 694 chemotherapy patients who met the inclusion criteria in our hospital.Among all investigated cases,the use problems of chemotherapy anti-emetic drugs was mainly identified in the seven aspects,including 50.86%(353/694)unsuitable choice of chemotherapy anti-emetic regimens,2.16%(15/694)inadequacy dose of 5-hydroxytryptamine-3 receptor(5-HT3-RAs),4.18%(29/694)inappropriate dose of dexamethasone,62.82%(436/694)over-or under-treatment of5-HT3-RAs,30.26%(210/694)treatment absence of dexamethasone,50.86%(353/694)unfavorable use of metoclopramide,36.74%(255/694)excessive use of proton pump inhibitors.Conclusion The results suggest a need for intervention to curb the irrational utilization of anti-emetic drugs in prescribing in this investigated hospital.The major measures for the rational use of chemotherapy anti-emetic drugs include standardized chemotherapy anti-emetic schemes,strict formulation of various doses of chemotherapeutic anti-emetic drugs,meticulous limitation of the frequency of drug delivery for anti-emetic drugs and the course of use of various chemotherapeutic drugs.Based on the above survey results,we analyzed that the main causes of irrational utilization of drugs were lack of mastering standard treatment guidelines and lack of continuing medical education for doctors and pharmacists.The results of this study are important for decision-makers to utilize when putting policies and strategies to improve the utilization of drugs in one hospital.We should enhance the management of anti-emetic drugs use to improve the quality of life and ensure the smooth progress of chemotherapy for patients.
