Objective:The effects of the maxillary molar inflammation on the maxillary sinusitis and the influence of the selection of planting time after extracting the tooth on the success rate of the implant are discussed through the analysis of CBCT images.Methods:The study analyzed CBCT images of 73 patients with inflammatory teeth.1、The relationship between maxillary molar inflammation and maxillary sinus floor was divided into three groups to observe the correlation.Group A: inflammation entered the maxillary sinus;group B: inflammation was connected to the maxillary sinus;group C: there is a certain distance between inflammation and maxillary sinus.2、In order to analyze the relationship between the planting time and the success rate of planting after extracting the maxillary molars,the planting time after extraction was divided into three groups,the immediate planting group,the early planting group and the delayed planting group respectively.The length of vertical bone resorption of implants by CBCT coronal surface was not more than 1/3 of the length of implant in bone as an imaging criterion for successful implantation.Results:In the experiment,the incidence of maxillary sinusitis in group A was 100%,the incidence rate of group B is 77.1%,and the incidence rate of group C is 42.9%.There were significant difference between three groups(P<0.05).There were 15 cases of maxillary sinusitis after the removal of the focus teeth,but the thickening of the fluid or mucous membrane was better than that before the extraction of the teeth.The success rate of immediate implantation after extraction of maxillary molars was 75%,and the success rate of early planting and delayed planting was 100%.The difference between the groups were statistically significant(P<0.001).There was a significant difference between immediate planting and delayed planting(P<0.01).There was no significant difference between immediate implant and early implantation,early implantation and delayed implantation.There were 2 implants falling off,all of them were immediate planting cases.And the other cases were normal in CBCT images.The vertical bone absorption of the implants in the CBCT coronal position were not more than the 1/3 of the implant in the bone.Conclusion: 1.The smaller the distance between inflammation and maxillary sinus,the greater the risk of maxillary sinusitis.2.The maxillary sinus has a certain ability to repair.The removal of the focus teeth is beneficial to the restoration of maxillary sinusitis.3.The success rate of immediate implantation after extracting the inflammatory maxillary molars is lower than that of early planting and delayed implantation.4.CBCT can directly and accurately evaluate the implant operation before operation,postoperative evaluation and postoperative follow-up,and can clearly reflect the changes of maxillary sinus.
