ObjectiveTo evaluate the diagnostic value of ultrasound-guided fine-needle aspiration(US-FNA)and core needle biopsy(US-CNB)in the diagnosis of thyroid nodules.MethodsA total of 396 patients(396 nodules)with thyroid nodules were examined by ultrasound in our hospital,all of whom underwent ultrasound guided thyroid nodule puncture.Patients were randomly divided into two groups:fine-needle aspiration group(n = 213)and core needle biopsy group(n = 183).The results of puncture biopsy were classified according to the Bethesda system for reporting thyroid cytopathology,and compared with the results of histology and pathology.The rate of postoperative bleeding,satisfaction rate and diagnostic coincidence rate of the two methods were compared.Results1.The postoperative bleeding rate in core needle biopsy group(6.56%)was higher than that in fine-needle aspiration group(1.88%).The difference was statistically significant(P= 0.019 < 0.05).2.The satisfaction rate of core needle biopsy group(97.81%)was higher than that of fine-needle aspiration group(92.02%).The difference was statistically significant(P=0.010< 0.05).3.The diagnostic coincidence rate of core needle biopsy group(95.48%)was higher than that of fine-needle aspiration group(92.55%),and there was no significant difference between two groups(P=0.241>0.05).4.The sensitivity,specificity,positive predictive value,negative predictive value,false negative rate and false negative rate of US-FNA in diagnosis of malignant thyroid nodules were 92.73%,92.48%,83.61%,96.85%,7.27% and 7.52%,respectively.The sensitivity,specificity,positive predictive value,negative predictive value and false negative rate of US-CNB in diagnosis of malignant thyroid nodules were 91.53%,97.46%,94.74%,95.83%,8.47% and 2.54%,respectively.There was no significant difference between US-FNA and US-CNB(P>0.05).ConclusionCompared with US-CNB,the satisfied rate of thyroid US-CNB is higher than US-FNA.However,US-FNA is a reliable method for the diagnosis of benign and malignant thyroid nodules.
