| Objective:It is mainly to explore the CT imaging features and the data of lesions and solid components and the pathological subtypes of lung adenocarcinoma(in situ adenocarcinoma(indecacinoma in situ)in patients with mixed ground glass nodule lesions less than or equal to 3 cm under double lung CT.,AIS),correlation between minimally invasive adenocarcinoma(MIA)and invasive adenocarcinoma(IAC)Data collection and methods:Retrospective collection of CT images from 102 patients with solitary mixed ground glass nodules at Dalian Munioipal Central Hospital from October 2016 to October 2018,All patients were due to a physical examination or a disease-independent Symptoms were found on chest CT examinations.All CT images were measured by a single-blind method in the lung window(window position,-700~-500 HU;window width,1,500~2 000 HU).The maximum diameter of the solid component,the proportion of solid components is calculated.At the same time,check the relevant imaging features such as the location of the nodule,the shape of the nodule,whether there are burrs or lobes on the edge,whether there are empty cannons and signs of pleural indentation.At the same time,postoperative pathological specimens were collected and classified according to the 2015 WHO classification of lung tumors.Data were collected and analyzed using SPSS 22.0 software.The maximum diameter and solid components of the lesions and solid components accounted for the independent sample T test,and the related CT image signs were analyzed by chi-square test.Statistical results were statistically significant at P<0.05.At the same time,the ROC curve of the receiver operating characteristic of IAC was plotted according to the maximum diameter of the lesion and the solid component of the IAC and the percentage between the two.The ROC curve was used to analyze the maximum diameter and solid component ratio of lesions and solid components in pathological diagnosis as the best cutoff value of IAC.Result:Among the 102 samples,the number of male was 43,and the female was 59;the mean age was 57.1±10.1 years before the age of 30 to 81 years old;20 cases were in situ adenocarcinoma(AIS),accounting for 19.6%;microinvasive gonads Cancer(MIA)29,accounting for 28.4%;invasive adenocarcinoma was 53,accounting for 52.0%.The distribution of lung adenocarcinoma in the lungs of the lungs was:35,10,and 15 cases in the upper,middle,and lower lobe of the right lung,and 23 and 19 in the upper and lower leaves of the left lung.The shape of the lesion under the enhanced CT(circular or elliptical and irregular shape),burr sign,lobulated sign,pleural indentation and air bronchus were statistically significant(P<0.05);but the vacuole sign was in this time.There was no statistical significance in the study(P=0.114>0.05).The maximum diameter of lesions and solid components was statistically different between the two pathological subtypes(P<0.05),while the ratio of solid components was not statistically significant between AIS and MIA.Difference(P=0.804>0.05).According to the IAC drawing in IMC of mGGN,the best cutoff values of the maximum diameter and solid component of the lesion and solid components were 16.5mm,5.5mm and 47.0%,respectively.Conclusion:The lung-enhanced CT has a statistically significant difference in the maximum diameter of lesions and solid components in different pathological subtypes of lung adenocarcinoma at≤3 cm mGGN.Enhanced imaging imaging features such as lesion shape,burr sign,lobulated sign,pleural indentation and air bronchus were also statistically different in distinguishing pathological subtypes of lung adenocarcinoma.At the same time,the ROC curve of IAC patients suggests that when the lesions,solid components and solid components account for 16.5mm,5.5mm and 47.0%,respectively,the pulmonary nodules should be alert to invasive adenocarcinoma,and their respective sensitivity And the specificity is best.However,this time,it is considered that there is no statistical difference in the degree of infiltration of the vacuolar sign in distinguishing lung adenocarcinoma. |
