Objectives:In this study,we use the adaptation mode model and the stress and coping model as the theoretical framework to understand the status of caregiver burden of the main caregiver for children with hand and foot congenital malformation,analysis its influence factors,put forward corresponding suggestions to reduce caregiver burden of the main caregiver for children with hand and foot congenital malformation,in order to provide theoretical basis for the intervention of clinical medical staff to reduce caregiver burden of the main caregiver for children with hand and foot congenital malformation.Method:This is a cross-sectional study,using convenient sampling method,taking 178 main caregivers for children with hand and foot congenital malformation from Hand and foot surgery wards of three first-class hospitals in Jinan as the research object.This study adopts the questionnaire method,including artificial General demographic data and clinical data of patients,Zarit Caregiver Burden Interview(ZBI),Social Support Rating Scale(SSRS),Simplified Coping Style Questionnaire(SCSQ),Positive Aspects of Caregiver(PAC)to investigate the level of caregiver burden of the main caregiver for children with hand and foot congenital malformation.Collected data were analyzed by SPSS 23.0 software.The statistical methods included descriptive statistics,T test,one-way ANOVA,Pearson correlation analysis and multivariate linear regression analysis.Result:(1)The total score of caregiver burden of the main caregiver for children with hand and foot congenital malformation was(25.89±14.29)points,and among the two dimensions,personal burden score was(15.96±7.88)points and liability burden was(5.81±5.11)points.Among them,41.6%of caregivers had mild burden,14.0%had moderate burden,and 3.4%had severe burden.(2)The total score of social support was(41.96±7.59)points,of which 9.33±3.08 points was objective support,and 25.35±5.22 points was subjective support,7.29±1.62 was support utilization points.The social support of the main caregiver for children with hand and foot congenital malformation accounted for 61.2%in general,0.56%in low social support and 38.2%in high social support.(3)Positive coping score was(20.69±6.31)points and negative coping score was(8.59±4.29)points in the main caregiver for children with hand and foot congenital malformation.(4)The score of positive aspects in the main caregiver for children with hand and foot congenital malformation was(32.96±7,46)points,self-affirmation score was(17.93±4.61)points and life outlook score was(15.03±3.36)points.(5)The results of t test and one-way ANOVA showed that there were significant differences in caregiver burden of the main caregiver for children with hand and foot congenital malformation(P<0.05)with marital status,monthly family income,malformation type and whether to continue surgery.(6)Pearson correlation analysis showed that social support,subjective support,positive coping,positive aspects,self-affirmation,life outlook were negatively correlated with caregiver burden of the main caregiver for children with hand and foot congenital malformation(P<0.05).Negative coping was positively correlated with caregiver burden(P<0.05).(7)Multivariate linear regression results showed that marital status,monthly family income,subjective support,negative coping,self-affirmation,life outlook and malformation type explained a total of 41.1%of the total variation of caregiver burden of the main caregiver for children with hand and foot congenital malformation.Conclusion:(1)The level of caregiver burden of the main caregiver for children with hand and foot congenital malformation was at the level of mild burden,and 59%of the main caregivers of children still had different levels of caregiver burden,in urgent need of reduction.(2)The level of caregiver burden of the main caregiver for children with hand and foot congenital malformation was affected by marital status,monthly family income,malformation type and whether to continue surgery.According to the main caregiver with spouse,complex type of malformation,and continuing to repair and monthly income were lower,the higher the level of caregiver burden was.(3)Social support,coping style,positive aspects can affect caregiver burden of the main caregiver for children with hand and foot congenital malformation.The better the social support,the more positive coping and positive aspects was,the caregiver burden of the main caregiver is lower.(4)Marital status,monthly family income,subjective support,negative coping,self-affirmation,life outlook and malformation type is a significant predictor of caregiver burden of the main caregiver for children with hand and foot congenital malformation.
