Abstract/Summary:
Objective:The pathogenesis of Parkinson’s disease is not completely obvious.With the development of the disease,drug therapy is facing great challenges.Therefore,in order to explore non-invasive and effective treatment methods,the purpose of this study is to understand the effect of cTBS treatment on motor symptoms and persistence of Parkinson’s disease patients.Objective to investigate the specific effects of cTBS on cortical excitability in patients with Parkinson’s disease,and to provide theoretical basis for clinical selection of cTBS in the treatment of motor symptoms in patients with Parkinson’s disease.Methods:From March 2016 to March 2017,15 patients with PD were selected from the Department of Neurology,Second Affiliated Hospital of Zhejiang University Medical College.General data such as sex,age,age of onset,course of disease and medication were collected.True and false stimulation of cTBS was performed on PD patients.The interval between the two stimuli was 2 months.Motor symptoms were assessed one day before stimulation,immediately after stimulation for one course and two weeks after stimulation.Cortical excitability was assessed one hour before stimulation and immediately after stimulation for one course,and the differences before and after treatment were compared.The UPDRS motor score,timing exercise test and 10-meter reentry exercise test were evaluated before treatment,immediately after treatment and 2 weeks after treatment.Results:1.Cortical excitability:The latency and resting period of cortex were prolonged,the resting threshold was increased,and the maximum amplitude of Ml region was decreased,but the difference was not statistically significant.2.Motor symptoms:The score of movement part of UPDRS before treatment was significantly higher than that after treatment and 2 weeks after treatment(39.00 ± 10.39 vs 25.80 ± 9.49 vs 26.80 ±10.28,p<0.01),including rigidity(28.80 ± 8.58 vs 20.40 ±7.67 vs 21.20 ± 7.56,p<0.05),postural gait part of UPDRS changed significantly(7.20± 2.94 vs 4.20 ± 2.04 vs 4.20 ± 2.68,p<0.05).Timing exercise after right hand treatment was significantly higher than that before treatment,and there was no significant difference in left hand.The data of 10-meter reentry movement before treatment were significantly slower than those after treatment and 2 weeks after treatment(18.12 ± 0.58 vs 16.89 ± 0.65 vs 17.49 ± 0.50,p<0.05).Conclusion:After treatment with cTBS,cortical excitability of PD patients decreased,but there was no statistical significance.cTBS treatment can improve the movement symptoms of PD patients,especially stiffness and postural gait symptoms,cTBS can effectively shorten the 10-meter reentry movement time of PD patients,improve the walking ability of patients,suitable for clinical application.
