Objective This study determined the levels of interleukin-18(IL-18)and ADA(adenosine deaminase)in pleural effusions of patients with tuberculous pleurisy and the changes of IL-18 levels after antinuclear treatment.To explore the possibility of application of IL-18 in the diagnosis of tuberculous pleurisy and treatment.Methods The clinical data of 20 patients with primary tuberculous pleurisy and 20 patients with non-tuberculous pleurisy(including pneumonia,purulent,etc.)who were initially diagnosed at the Department of Thoracic Surgery,Oriental Hospital,Lianyungang City,Jiangsu Province,from December 2016 to June 2017 were analyzed.The levels of IL-18 in pleural effusions from patients with tuberculous pleurisy and those with nontuberculous pleurisy were determined using a double-antibody sandwich method.ADA was measured by enzyme-rate method.The levels of IL-18,ADA,and IL-18 levels after treatment were compared between the two groups.SPSS22.0 statistical software was used for statistical analysis.The measurement data was represented by(S).X2 was used to test the count data.For the homogeneity analysis of the variance of the data,the variance of the variance of the normal distribution was taken as the t test to compare the two groups.P<0.05 was considered statistically significant.Results The average level of interleukin 18 in pleural effusion was(520.5 ± 42.6)pg / m L in tuberculous pleurisy patients and 174.5 ± 12.7 pg / m L in non-tuberculous pleurisy patients(P <0.05).The levels of ADA in the tuberculous pleurisy group were(50.2 ± 13.2)U / L on the average and(12.1 ± 11.1)U / L in the non-tuberculous pleurisy group,respectively.In patients with tuberculous pleurisy,The level of IL-18 in pleural effusion dropped to(425.5 ± 27.5)pg / m L,(368.4 ± 21.7)pg / m L,(208.9 ± 15.9)pg / m L on 3,After 7 days of treatment,it was significantly lower than that at admission(P <0.05).Conclusion 1.Compared with other inspection methods,it is easier to be accepted by the patient.Can be used as an indicator of early diagnosis.2.The levels of IL-18 in the two groups of pleural effusions are different,suggesting that IL-18 is not only involved in the pathological process of tuberculous pleurisy.3.but also can be used as an immunological indicator for early diagnosis and observation of therapeutic effects.
