OBJECTIVE: In this paper,we study the portal vein thrombosis after liver resection was compared with the pure liver surgery: a system evaluation and meta analysis,to study the curative effect after operation method for the liver resection,to clinical doctors choose operation method to provide more guidance and significance of evidence-based medicine.Materials and Methods: This paper uses the pubmeda cochrane librar, medline database to retrieve the literature,To search the relevant literatures of hepatectomy after portal vein embolization(portal vein embolization without combined hepatic arterial chemoembolization and chemotherapy)and simple hepatectomy. The combination of "portal vein embolization","hepatectomy","liver resection" and "hemihepatectomy" was used as the subject word or key word. The related data were analyzed by meta using review manager 5.3.RESULTS: A total of 16 literatures were included in the meta-analysis,with a total of 1,784 patients, including 564 cases of liver resection after portal vein embolization, and 1220 patients with simple resection. The survival rate,R0 removal rate, postoperative complications and length of hospitalization were not statistically significant(P > 0.05)compared with that of simple hepatic resection in 1 year,3 years and 5 years. The surgical time and the incidence of bile leakage were lower than the postoperative hepatic resection, and the difference was statistically significant(P < 0.05).The incidence of hepatic failure was lower than that in the hepatectomy group, and the difference was statistically significant(P < 0.05).CONCLUSIONS: Based on the retrieved literature, and the Meta analysis showed that patients with portal vein thrombosis after liver resection with pure liver resection patients 1 year,3 years and 5 years survival rate,R0 resection rate, there was no difference in total complications and length of hospital stay, postoperative portal vein thrombosis in patients with liver resection surgery time is longer than pure liver resection patients, patients with portal vein thrombosis after liver resection patients with bile leakage rate is higher than pure liver resection, lower incidence of portal vein thrombosis after liver resection in patients with liver failure patients with liver resection alone.
