Background:In recent years,with the development of diagnosis technology and the increased awareness of intraductal papillary mucinous neoplasm(IPMN),the reports of IPMN are rising.In view of the treatment and prognosis of benign and malignant IPMN are different,the preoperatively evaluation of the malignant degree of IPMN is necessary.This study by analyze the clinicopathological characteristics of IPMN to explore its malignant risk factors.Methods: A retrospective study was carried out on 34 patients with IPMN between Jan.2013 to Dec.2016.According to the World Health Organization's pathology classification standard,all patients were divided into two groups: benign group(IPMN with low-or intermediate-grade dysplasia)and malignant group(IPMN with high-grade dysplasia and invasive carcinoma).By statistically analyze the relationship between clinicopathological characteristics and the extent of benign or malignancy,exploring malignant risk factors of IPMN.Results: The 34 cases' age onset vary from 35 to 84 years old(median age,67.5 years),2 cases ?50 years old,while 32 cases >50 years old;24 cases are male and 10 cases are female;15 cases' tumor locate at the head of pancreas,17 cases' locate at the body and tail and 2 cases' whole pancreas are involved;Histological review reveals 18 cases of low-or intermediate-grade dysplasia,7 cases of high-grade dysplasia and 9 cases of invasive carcinoma;There 12 cases are main duct type,18 cases are branch duct type and 4 cases are mixed type.The univariate analysis shows main pancreatic duct(MPD)?7.25mm(p=0.003),serum CA19-9 level?23.65 U/mL(p=0.047)and Ki67 index ?12.2%(p<0.001)are indicators of malignant IPMN.Conclusions: MPD?7.25 mm,serum CA19-9 level?23.65 U/mL may become prediction index of malignant IPMN preoperatively;Ki67 index ?12.2%may become prediction index of IPMN's prognosis after surgery.
