The implantation of anterior teeth requires both the chewing function and good aesthetic effect.However,as the loss of anterior teeth,the alveolar bone atrophy occurs rapidly,resulting in insufficient local bone volume,which makes it difficult for implant surgery.Therefore,bone augmentation is necessary during implant surgery.At present,the most common bone augmentation technique is GBR(Guided Bone Regeneration).GBR means the bone volume increasing by using biological barrier membrane and bone substitute materials.The barrier membrane can prevent the soft tissue from growing into the bone defect area,and provide the room for bone growth.Generally,bone substitutes have good ability of bone conduction,lacking the ability of induction.So promoting the ability of bone induction of bone substitute is the tendency for clinical research.For severe bone defect,titanium mesh is an appropriate material as the barrier.However,the rigidity of titanium mesh may lead to compromised wound healing.In this research,we use a new blood-derived bio-material(platelet-rich fibrin,PRF)combined with bone substitute and titanium mesh,for better GBR result.We observed clinical affects and study on the possibility and advantages of the technique,in order to provide theoretical basis for clinical use.Materials and methods:13 aesthetic area missing tooth cases were selected and divided in two groups.One group(6 cases)was performed dental implantation combined with pure artificial bone implantation covered by titanium mesh.The other group(7 cases)was performed dental implantation combined with artificial bone mixed with PRF implantation covered by titanium mesh and PRF membrane.Wound healing was evaluated at the 14th day after operation.After 4 and half months post-operation,newly formed bones are harvested for histological observation.Results:Wounds of PRF group were well healed without inflammation and swelling.In contrast,one case of GBR group had a satisfied wound healing.Three cases had wound healing with inflammation and swelling.Two cases had wound dehiscence with the exposure of titanium mesh.For histological observation,PRF group showed more matured bone formation with more trabecular bones.Conclusion:Using of PRF and titanium mesh can improve the newly bone formation and reduce the tissue reaction,thus showing a satisfied clinical value in GBR procedure.
