The Clinical Characteristics And Treatment Analysis Of Pediatric Patients With Hepatic Cystic Echinococcosis

Objective:To investigate the clinical characteristics,diagnosis and treatment ofhepatic cystic echinococosisin children.Methods:152 pediatric patients with age from1 to 14 year old and various clinical histories,were included in this study.The clinical date of 152 pediatric patients with hepatic cystic echinococcosis who treated in the first affiliated Hospital of Xinjiang Medical University were analyzed.The Clinical characteristics,manifestations,types,diagnosis and treatment of them were retrospectively reviewed.Results:All patients included 52 cases of intrahepatic multiple cystic echinococcosis,26 cases of hydatid disease combined with other organs,4 cases of recurrent echinococcosis.139 patients underwent surgery,in witch 97 cases were treated with removal of internal capsule + external capsule subtotal resection,18 cases were for total pericystectomy and 34 cases were for Simple internal capsule extirpation.3 patients who had been treated only with drugs.Most of the sites were located in the right half liver and were found in the right epigastric pain,abdominal distention or rhysical examination.149 patients were operated on and 3 patient were treated only with drugs.All patients were followed up for 1-2years after treatment.The patients who were treated surgically were treated without recurrence.However,19 cases of postoperative residual cavity effusion were found,among which 5 cases were complicated with infection,and then treated with drugs.Among the patients who were treated with drugs,2 were given surgical treatment after 6 months and 1 was recommend continued drug therapy.Conclusion:Pediatric live hydatid disease usually found inadvertently or in the physical examination,which early appear as abdominal painless mass.Hydatid cyst in children has the characteristics of fast growth,easy occurrence of multiple haydatid cyst,multiple organ echinococcosis and easy rupture of hydatid cyst.Surgical treatment is primary,drug therapy is secondary.According to the different growth parts of echinococcosis,the first choice of the surgical methods is different.Removal of internalcapsule+external capsule subtotal resection is the best choice for exogenous hepatic cystic echinococcosis,and simple internal capsule extirpation is the first choice for endogenetic hepatic cystic echinococcosis.
