| Objective: To investigate the first influencing factors of depression in patients with solitary pulmonary nodules,in order to take effective measures to prevent and treat patients’ psychological problems and to achieve better treatment results and improve the quality of life of patients.Methods:Patients with pulmonary nodules who were first diagnosed in our Hospital Thoracic Surgery from October 2016 to December 2018 were selected.All patients were excluded from the history of mental and personality disorders,and no adverse social factors were affected.The educational level of primary school and above was voluntarily coordinated with the investigation.We used the Zung Depression Self-Assessment Scale(SDS)to investigate patients.The general condition and corresponding factors of the patient include: age,gender,ethnicity,blood type,patient’s awareness of the disease,spouse status,patient’s attitude towards death,support of family and friends,support of children to patients,family income,past History of smoking(smokers,quitters or never smokers),alcoholism(twice a week or more),comorbidities(including coronary artery disease,arthritis,diabetes,history of tuberculosis,hypertension,obstructive pulmonary disease,etc.),education Level(divided into four grades: elementary school,junior high school,high school,college and above),whether surgery or not.And to analyze the risk factors affecting depression in patients with newly diagnosed pulmonary nodules.Results: The results of SDS scale showed that 47(42.7%)of 110 newly diagnosed pulmonary nodules had depression(> 50 points).Among them,39 cases(35.5%)had mild depression,7 cases(6.3%)had moderate depression and 1 case(0.9%)had severe depression.Univariate analysis showed that age,family income,whether through surgical treatment,patients’ own awareness of the disease,family and friends’ support for patients,patients’ attitude towards death,spouse and children’s support for patients,patients with diabetes mellitus and other related factors had a significant impact on the occurrence of depression in patients with newly diagnosed pulmonary nodules(P <0.05).Multivariate logistic regression analysis showed that age,disease awareness,attitude to death and diabetes mellitus were risk factors for depression in newly diagnosed pulmonary nodules(OR = 2.625,1.635,3.462,2.431),while spouse and child support,surgical treatment and family income were protective factors(OR =0.552,0.681,0.765,0.514).Conclusion: The incidence of depression in patients with pulmonary nodules at first diagnosis was high,and it was affected by many factors.Psychotherapy should be emphasized in order to improve the therapeutic effect and quality of life of patients. |
