Objective: To compare the difference of metabolic components among papillary thyroid carcinoma,benign thyroid nodules and non-thyroid nodules.Methods: The clinical data and fasting serum samples of patients who underwent thyroidectomy at thyroid surgery department and individuals who had no thyroid nodule in Health Management Center were collected in the Second Affiliated Hospital of Dalian Medical University from May to October 2017.According to the postoperative pathological,it was divided into papillary thyroid carcinoma group(118 cases)and benign thyroid nodule group(84 cases).At the same time,those(98 cases)who with no thyroid nodules,normal thyroid function and negative autoantibodies were taken as the normal control group.The sex,age,body mass index(BMI),fasting plasma glucose(FPG),fasting insulin(FINS),homeostasis model assessment of insulin resistance index(HOMA-IR),total cholesterol(TC),triglycerides(TG),low density lipoprotein chole-sterol(LDL-C),high density lipoprotein cholesterol(HDL-C),free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH),antithyroglobulin antibodies(TGAb)and antithyroid peroxidase antibodies(TPOAb)were compared among the three groups.Furthermore,the risk factors of papillary thyroid carcinoma were determined by multivariate logistic regression analysis.Results:1.Among 202 patients with thyroid nodules,there were 118 cases with papillary thyroid carcinoma group which included 87 females(73.73%)and 31 males(26.27%)and 84 cases with benign thyroid nodules which consisted of 60 females(71.43%)and24 cases males(28.57%)according to postoperative pathological results.In addition,41 females(41.84%)and 57 males(58.16%)were 98 cases with normal control group.Compared with the normal control group,the proportion of female in papillary thyroid carcinoma group and benign thyroid nodule group was obviously higher than that of male respectively(P<0.05).However,there was no significant difference in theproportion of male and female between papillary thyroid carcinoma group and benign thyroid nodule group(P>0.05).2.The average age of papillary thyroid carcinoma group and benign thyroid nodule group were larger than that of normal control group(P<0.05),and the average age of papillary thyroid carcinoma group was lower than that of benign thyroid nodule group(P<0.05).Similarly,the BMI level in papillary thyroid carcinoma group was significantly higher than that in normal control group(P<0.05),and no significant difference between papillary thyroid carcinoma group and benign thyroid nodule group(P >0.05).3.The FPG level in papillary thyroid carcinoma group and benign thyroid nodule group were higher than that in normal control group respectively(P<0.05),and no significant difference between papillary thyroid carcinoma group and benign thyroid nodule group(P>0.05).The FINS level in papillary thyroid carcinoma group was higher than that in normal control group(P<0.05),but was no significant difference when compared with benign thyroid nodule group(P>0.05).Similarly,the HOMA-IR of papillary thyroid carcinoma group was higher than that of normal control group(P<0.05),but was no significant difference when compared with benign thyroid nodule group(P>0.05).4.The TGAb levels of papillary thyroid carcinoma group and benign thyroid nodule group were higher than that of normal control group(P<0.05).No significant differences were found in the TGAb and TPOAb levels between papillary thyroid carcinoma group and benign thyroid nodule group(P>0.05).5.No significant differences were discovered in the TC,TG,LDL-C,HDL-C,FT3,FT4 and TSH levels among the three groups(P >0.05).6.Multivariate logistic regression analysis showed that papillary thyroid carcinoma was negatively correlated with age and positively correlated with the TGAb level(P<0.05).Conclusion:1.Comparing with normal control group,the proportion of female is significantlyhigher than that of male in papillary thyroid carcinoma group and benign thyroid nodule group respectively.Nevertheless,there is no statistical difference in sex between papillary thyroid carcinoma group and benign thyroid nodule group.2.The average age of papillary thyroid carcinoma group and benign thyroid nodule group are significantly higher than that of normal control group.But the average age of papillary thyroid carcinoma group is lower than that of benign thyroid nodule group,there is no significant difference between two groups.3.The FPG,FINS and HOMA-IR levels in papillary thyroid carcinoma group and benign thyroid nodule group are significantly higher than those in the normal control group.4.The age and TGAb level may be risk factors of papillary thyroid carcinoma.
