Study On The Predisposing Factors Of Community Acquired Pneumonia In Children With Special Constitution

Objective: Based on the theory of TCM constitutional physiology,this paper investigates and analyzes the relationship between children with special constitution and community acquired pneumonia in Guangxi,and explores the predisposing factors of community acquired pneumonia in children with special constitution in Guangxi,and provides clinical evidence-based evidence for prevention and treatment of community acquired pneumonia in children with special constitution.Methods: Refer to "Children's TCM Constitution Identification Questionnaire" by adopting the "Traditional Chinese Medicine Physique Classification and Judgment" standard issued by the Chinese Medicine Association and combining with Wang Xiaoming's "Healthy Children's TCM Constitution Identification Method".The questionnaire includes the identification points of the physical characteristics,physique,susceptibility to diseases,psychological characteristics,adaptability,etc.Draw up the "conditions for judging the physique of pediatric traits" according to the results of the national pediatric clinical expert's questionnaire.It includes the quantitative indicators of the presence and frequency of common symptoms,the past history of special constitution,family history,environmental adaptability and special signs etc;Work out the “Pneumonia Susceptibility Factors Questionnaire”,which covers the basic information of the respondents,whether they have had community acquired pneumonia,birth conditions,diet feeding sleep and activities,past illness and family background,and other factors that may affect the community acquired pneumonia.During April2017 to September 2018,using the method of clinical questionnaire survey,recruited children age from 1 to 6 who come from Cenxi City,Liuzhou City,Beihai City,Sanjiang County and Nanning City as the research objects.According to the frequency of occurrence of "no,very few,sometimes,often,always",etc.,the child's physical fitness index is quantified,and the total score and calculation of the items are judged by each physical quality to determine the physical type and physical bias tendency;It was grouped and counted according to the survey Children's physical type and the history of community acquired pneumonia.The chi-square analysis was performed on whether there was a history of CAP in children with special constitution and whether there was a history of CAP in non-special children.Take the basic situation,whether you have had CAP,birth conditions,diet and feeding,sleep,participation in sports,past history,family influence factors,passive smoking,etc.altogether 39 possible prone to CAP and physical type as the independent variables,and CAP as a dependent variable,to perform logistic regression analysis and COX regression analysis on children with special constitution.Results: A total of 466 valid questionnaires were collected,of which 343 were special constitution,including 104 cases of special constitution and heat accumulation,66 cases of qi deficiency,38 cases of yin deficiency and 16 cases of phlegm and dampness.There were 13 cases of yang deficiency.343 children with special constitution,214 cases with CAP history(accounting for72.4%),129 cases without history of CAP(27.6%),123 cases with non-special constitution,including 38 cases of CAP(30.9%),no A history of CAP in 85 cases(accounting for 69.1%);According to the logistic regression analysis,it was found that children with CAP and previous asthma,repeated respiratory infections,basic diseases such as malnutrition,the history of hospitalization,the antibiotic use were statistically significant(P<0.05);the number of CAP in children with special constitution was related to phlegm and blood stasis;COX regression analysis showed that the initial age of CAP in children with special constitution was related to AD/OL of congenital heart disease,malnutrition,passive smoking history,and lower eyelid black sign.The predisposing factors for children with CAP are: previous asthma,repeated respiratory infection,malnutrition and other basic diseases,past hospitalization history,the antibiotics use in history.Conclusion: The presence of asthma,repeated respiratory infections,bronchitis and use of antibiotics are the risk factors for CAP in children with special constitution.Children with malnutrition and passive smoking have a younger age of CAP.The more obvious the black mark of children's lower eyelids(high AD/OL value),the earlier the age of CAP is the first time;the children with special constitution and phlegm-dampness have higher CAP than children of other constitution types.
