Objective Malignant tumors are the second most serious disease in humans.Every year,tens of thousands of patients suffer from neoplastic diseases.The number and types of anti-tumor drugs used in clinical practice are complex,and cytotoxic drugs are mostly.The tumor itself is prone to bleeding,and anti-tumor drugs will inevitably interact when combined with warfarin.Care should be taken when using them together to avoid bleeding complications and affect the therapeutic effect.As an anticoagulant,warfarin is widely used in clinical practice,and its anticoagulant effect is good,and it is often used as the drug of choice.However,due to the narrow therapeutic window of warfarin,it is easy to interact with other drugs,so the effect of interaction on the efficacy should be considered when clinically combining warfarin.At present,domestic and foreign scholars have also carried out research on the interaction between anti-tumor drugs and warfarin,and obtained the results of interaction between certain anti-tumor drugs and warfarin.This article studied the drug use of warfarin in patients with tumors in our hospital,verified and classified the interaction level between chemotherapy drugs and auxiliary drugs and warfarin,observed the INR value of patients,and used for clinical tumor chemotherapy combined with warfarin.Methods From January 2017 to December 2018,66 patients with newly diagnosed oncology were enrolled in the Oncology Department of The Firet Affiliated Hospital of Zhengzhou Universiey,aged 11 to 76 years.There are 37 male patients and 29 female patients.The tumor pathology types of all patients,the use of chemotherapy drugs,the classification of adjuvant drugs,the cause of warfarin,the amount of warfarin,and the INR value were counted.Through MCDEX system to verify the level of interaction,risk and clinical drug recommendations of all tumor chemotherapy drugs and warfarin,verify the level of interaction,risk and clinical drug recommendations of all types of adjuvant drugs and warfarin;analyze the reasons for different warfarin drugs The warfarin dosage,medication adjustment,and adjusted INR values.All patients who were adjusted for the dose of warfarin were analyzed for the reasons for warfarin,chemotherapy,adjuvant,dose adjustment and INR.Special statistical analysis of the causes,dosages and complications of warfarin in patients with lung cancer.Results Among the selected patients,37 were male and 29 were female;most were lung cancer and non-Hodgkin’s lymphoma,14 were lung cancer patients,accounting for21.2%;16 were non-Hodgkin’s lymphoma,accounting for 24.2%.In this study,51.85% of chemotherapeutic drugs can enhance the effect of warfarin to varying degrees,and only cyclophosphamide can reduce the anticoagulant effect of warfarin.Most of the chemotherapy-assisted drugs in this study interacted with warfarin,including anticoagulants(9.68%),non-steroidal anti-inflammatory drugs(3.23%),hormones(6.45%),and acid-suppressing drugs(12.90).%),lipid-lowering drugs(6.45%),analgesics(6.45%),cardiovascular drugs(6.45%),antibacterials(9.68%),diuretics(9.68%)all have synergy with warfarin Diuretics can attenuate the effects of warfarin;antiemetics in MCDEX do not suggest interaction with warfarin.In this study,44 patients with deep vein thrombosis(VTE)were formed,accounting for 66.7%,and the dose of warfarin was 3.0±1.18 mg,QD;in patients with PE,the dose of warfarin was 2.5 mg-3.125 mg,and the average medication was given.The dose was 2.6 ± 0.24 mg,and a total of 6 patients adjusted the amount of warfarin.Conclusions Most tumor chemotherapy drugs and tumor adjuvant drugs interact with warfarin.In the clinical use of warfarin,it is necessary to regularly check the INR value,adjust the amount of warfarin,ensure the smooth progress of chemotherapy in cancer patients,and improve the treatment gain of patients.
