Objective:To analyze and summarize the characteristics of MRI findings in intraductal papilloma and to improve the understanding and diagnosis of MRI images of intraductal papilloma. Methods:A retrospective analysis of 94 cases of intraductal papilloma confirmed by pathology in the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine, according to whether or not with catheter dilation, all cases were divided into a group of ductal dilatation(58 cases)and a group without ductal dilatation(36 cases),comprehensive analysis of lesions and comparison between groups were made from lesion size, morphology,DWI signal intensity,ADC value,T1 pre-scan signal performance, lesion enhancement characteristics, and dynamic enhancement time-signal curve(TIC curve)types.Analyze, record and summarize. Results:There was a significant difference in the maximum diameter of lesions between the catheter dilation group and the non-catheter dilation group(P < 0.05).In DCE-MRI,75 cases(79.79%)showed early rapid arterial enhancement, and 19 cases(20.21%)showed early moderate or slow arterial enhancement. There was no significant difference in early enhancement rate between the catheter expansion group and the non-catheter dilation group(P > 0.05).There were 25 cases(26.60%)with homogeneous enhancement in the early stage of arterial enhancement and circular enhancement in the delayed central enhancement. There was no significant difference between the catheter expansion group and the non-catheter expansion group(P > 0.05).There were 16 cases(17.02%)with ring-like enhancement in both early and delayed arteries. There was no significant difference between catheter dilation group and non-catheter dilation group(P > 0.05).There were 22 cases(23.40%)with homogeneous or "target ring" enhancement in the early stage and "target ring" enhancement in the delayed stage. There was a significant difference between the catheter expansion group and the catheter expansion group(P <0.05).There were 22 cases(23.40%)with uniform enhancement in early and delayed arteries, and the difference between the group with and without catheter expansion was statistically significant(P<0.05).There were 9 cases(9.57%)with ductal or branching enhancement without nodules or masses, all of which occurred in the catheter dilatation group. There was no significant difference in TIC curve type between catheter dilatation group and non-catheter dilatation group(P > 0.05).Conclusions: Intravascular papilloma without ductal dilatation is generally larger than intraductal papilloma with ductal dilatation. The enhancement of intraductal papilloma is characterized by homogeneous or circular enhancement or "target ring" enhancement in the early stage of arteries, and circular enhancement or "target ring" enhancement in the delayed stage.Among them, delayed "target ring" enhancement is more common in ductal papilloma without ductal dilatation, and homogeneous enhancement in early and delayed arteries is more common in ductal papilloma with ductal dilatation.
