Objective: This article aims to understand the serum carcinoembryonic antigen(CEA)concentration of patients between primary and secondary pulmonary fibrosis,and to evaluate the correlation between serum CEA and degree of pulmonary fibrosis,so as to provide reference for clinical diagnosis and evaluation of diseases.Methods: The clinical data of 116 patients with pulmonary fibrosis admitted to the First Affiliated Hospital of Guangxi Medical University from March 2013 to November 2018 were retrospectively analyzed.According to the causes of pulmonary fibrosis,the patients were divided into idiopathic or secondary pulmonary fibrosis,idiopathic pulmonary fibrosis(IPF)group contain79 cases,connective tissue-related interstitial pneumonia(CTD-ILD)group include 37 cases,and 50 cases were randomly selected as healthy control group.The serum CEA?pulmonary function and chest high resolution CT(HRCT)data of each group were collected retrospectively as experiment data.The pulmonaryfunction indexes include forced vital capacity as a percentage of predicted value(FVC%pred),1s forced expiratory volume as a percentage of predicted value(FEV1%pred),1s forced expiratory volume as a percentage of vital capacity(FEV1/FVC),and carbon monoxide dispersion as a percentage of predicted value(DLCO%pred).Pulmonary fibrosis was scored according to chest HRCT data.Rank sum test was used to compare the differences of serum CEA concentration among the groups.Unconditional binomial Logistic regression analysis was used for factors of Serum CEA concentration elevated.Spearman correlation analysis was used to analyze the correlation between serum CEA concentration and pulmonary function and pulmonary fibrosis score.Results:1.The serum CEA concentration is the highest in IPF group,which is 4.15(2.16-7.09)ng/ml.The serum CEA concentration is2.74(1.99-4.48)ng/ml in CTD-ILD group.The serum CEA concentration is1.80(1.13-2.84)ng/ml in healthy control group.The serum CEA concentration in IPF group is significantly higher than that in CTD-ILD group and healthy control group,with P =0.014 ? 0.000.The serum CEA concentration in CTD-ILD group was higher than that in healthy control group,P=0.002.2.Serum CEA concentration is positively correlate with chest HRCT fibrosis score in IPF and CTD-ILD group,r=0.677,0.428,P=0.000,0.008.3.Serum CEA concentration is significantly negatively correlated with DLCO%pred in IPF group,P=0.000,correlation coefficient R =-0.507;Theserum CEA concentration was significantly negatively correlated with DLCO%pred in CTD-ILD group,P=0.005,r=-0.451.4.Unconditional binomial Logstic regression analysis showed that the increase of serum CEA was related to chest CT fibrosis score(P=0.000,OR=1.627,95%Cl=1.252-2.114)and DLCO%pred(P=0.024,OR=0.947,95%Cl=0.904-0.993).Conclusions: 1.Serum CEA concentration can be increased in IPF group and CTD-ILD group,being the most obvious in IPF group.2.Serum CEA concentration is positively correlated with pulmonary fibrosis in IPF and CTD-ILD patients,and it can reflect the severity of pulmonary fibrosis.
