PurposeTo explore the correlation between pulsatility index and risk factors in patients with cerebral white matter lesions caused by different degrees of middle cerebral artery stenosis,in order to provide a theoretical basis for clinical effective intervention.Method90 patients were collected from January to December 2018 in neurology clinics and wards that have improved TCD,head MRI,CTA and/or DSA related examinations.According to the MRI results,90 patients were divided into case group(group A)and control group(group B)according to the presence or absence of white matter disease using the Fazekas scoring standard.According to the degree of CTA/DSA vascular stenosis and NASCET grading criteria,the degree of vascular stenosis was divided into avascular stenosis group,mild vascular stenosis group and moderate-serve vascular stenosis group: A component was divided into A1(avascular stenosis group)and A2(slight vascular stenosis group),A3(middle-severe vascular stenosis group),B component was B1(avascular stenosis group),B2(mild vascular stenosis group)and B3(middle-severe vascular stenosis group).At the same time,general data of patients were collected: gender,age,history of hypertension,diabetes,hyperlipidemia,smoking and drinking;laboratory indicators: homocysteine,low density lipoprotein,total cholesterol,uric acid,etc;Transcranial doppler(TCD): statistics of the left/right middle cerebral artery pulsation index(MCA PI).Statistical analysis was performed using the spss24.0software package,and risk factors were analyzed using logistic regression,and multiple linear regression analysis was used to analyze the correlation.Result1.In the control group,patients with different grades of vascular stenosis had statistically significant differences in hypertension,diabetes,smoking,low-densitylipoprotein,and PI values,and were independent risk factors for middle cerebral artery stenosis(p<0.05).In the case group,the difference of hypertension and PI in patients with different grades of stenosis was statistically significant,which was an independent risk factor for cerebral white matter lesions caused by middle cerebral artery stenosis(p<0.05).2.Risk factors were correlated with PI values in 90 patients,hypertension,diabetes,smoking,low density lipoprotein were positively correlated with PI(p<0.05).Conclusion1.Hypertension,diabetes,smoking,low density lipoprotein and PI are independent risk factors for middle cerebral artery stenosis;Hypertension and PI are independent risk factors for cerebral white matter lesions caused by middle cerebral artery stenosis.2.High blood pressure,diabetes,smoking and low density lipoprotein are positively correlated with PI.
