Objective:The purpose of this study is to explore the relationship between CSVD and cognitive impairment,and to further analyze its impact on language function and its clinical significance.Methods:From May 2017 to December 2018 at the second hospital of Jilin University,Department of Neurology,90 patients who were diagnosed with cerebral small vessel disease and 20 healthy individuals as the control group were selected.According to the imaging manifestations,divided CSVD patients into LI group? WML group?CMBs group?LI+WML group ? LI+CMBs group ? WML+CMBs group and LI+WML+CMBs group.Minimum Mental State Examination(MMSE)?Montreal Cognitive Assessment(MoCA)and Western Aphasia Battery(WAB)were used to evaluate the degree of cognitive impairment and language impairment in these groups.SPSS19.0 statistical software was used for analysis,chi-square test ? t-test analysis and one-way ANOVA were applied in the count data and measurement data for analysis.Multi-variate logistic regression analysis wasapplied to analyze the risk factors of cerebral small vascular disease.There with p<0.05 was considered statistically significant.Results:1.There were 21 patients in LI group,19 in WML group,5 in CMBs group,26 in LI+WML group,6 in LI+CMBs group,5 in WML+CMBs group and 8 in LI+WML+CMBs group.There was no significant difference in age?sex?education years?smoking and drinking history between each group and normal control group.2.The prevalence of hypertension ? hyperlipidemia and diabetes mellitus in each group was significantly higher than that in the normal control group,and the difference was statistically significant.3.MMSE and MoCA score showed that the total scores in each group of CSVD were lower than those in the normal control group.The mean values of LI+WML+CMBs group were lower than those in LI group?WML group and CMBs group,the difference was statistically significant(p<0.05).4.WAB score showed that the mean values of LI group ? WML group?LI+WML group?LI+CMBs group and LI+WML+CMBs group were lower than those of normal control group,and the mean values of LI+WML+CMBs group were lower than those of WML group and CMBs group,with significant difference(p<0.05).5.Multi-variate logistic regression analysis showed thathypertension ? hyperlipidemia ? diabetes mellitus were related to the occurrence of cerebral small vessel disease,hypertension ?hyperlipidemia ? diabetes mellitus may be risk factors for CSVD.The scores of MMSE?MoCA and WAB in patients with CSVD were lower than those without CSVD.Conclusion:1.Different types of CSVD have effects on cognitive and language functions of patients.2.WML has greater impact on cognitive function and LI has greater impact on language function.3.Hypertension ? hyperlipidemia and diabetes mellitus may be associated with the development of CSVD.
