Objective This study selected healthy controls and moderate to severe stable stage COPD patients to analyze the levels of HLA-? and HLA? in different populations and their value in the assessment of COPD inflammatory response.Methods A total of 100 patients with stable stage COPD in the 9th hospital from September2015 to December 2017 were selected as the COPD group.50 healthy people in the physical examination department were selected as the healthy control group.Patients in the COPD group were divided into 50 patients with moderate COPD and 50 patients with stable COPD according to the degree of disease.General data such as gender,age,height,and weight of all subjects were recorded.The morning fasting elbow venous blood was taken from the subjects,and exhaled condensate was collected within 20 minutes after flushing the mouth with the RTube system.Enzyme-linked immunosorbent assay was used.The Enzyme-linked immunosorbent assayreader measures the concentration of sHLA-1 and sHLA-? molecules in serum and EBC at a wavelength of 405 nm.Forced expiratory volume in one second(FEV1),forced vital capacity(FCC),and FEV1/FVC were measured using a pulmonary function meter;Detection of sputum inflammatory cell counts in the subjects;Serum interleukin-6(?L-6),interleukin 8,?L-8,?nterferon-?(?FN-?),tumor necrosis factor-?(TNF-?)levelswere measured by an automated biochemical analyzer;C-reaction protein(CRP)levels were determined by immunoturbidimetry.Results:(1)EBC sHLA-?,Serum sHLA-?,EBC sHLA-?,and Serum sHLA-? were higher in the COPD group than in the healthy control group(P<0.05);EBCsHLA-?,Serum sHLA-?,EBC sHLA-?,and SerumsHLA-? were significantly higher in patients with severe COPD than in patients with moderate COPD(P<0.05).(2)There was a significant positive correlation between EBH-? and sHLA-? levels in EBC and serum(P<0.05).There was a significant positive correlation between EBC and serum sHLA-? and sHLA-? levels in patients with different severity of COPD(P<0.05).(3)The levels of FEV1,FVC and FEV1/FVC in COPD group were lower than those in healthy control group(P<0.05).The levels of FEV1,FVC and FEV1/FVC in COPD patients gradually increased with the severity of lung disease(P<0.05).There was a significant negative correlation between FEV1,FVC,FEV1/FVC and Serum sHLA-? in COPD class ? patients(P<0.05).There was a significant negative correlation between FEV1,FVC and EBC sHLA-?,Serum sHLA-?,EBC sHLA-? and Serum sHLA-? in patients with COPD grade ??(P<0.05).FEV1/FVC showed a significant negative correlation with EBC sHLA-?,EBC sHLA-?,and Serum sHLA-? levels(P<0.05).(4)The neutrophil count in the COPD group was higher than that in the healthycontrol group,and the lymphocyte and macrophage counts were lower than those in the control group(P<0.05).There was no significant difference in eosinophil count between the two groups(P>0.05).The neutrophil level of patients with COPD increased gradually with the severity of the disease,and the lymphocyte and macrophage counts gradually decreased with the severity of the disease(P<0.05).There was no significant difference in eosinophil count between the groups(P>0.05).The neutrophil counts of COPD class ? and COPD class ?? patients were positively correlated with EBC sHLA-?,Serum sHLA-?,EBC sHLA-? and Serum sHLA-?(P<0.05).Lymphocyte count was negatively correlated with EBC sHLA-?,Serum sHLA-?,EBC sHLA-?,and Serum sHLA-?(P<0.05).The macrophage counts of COPD class ? patients were significantly negatively correlated with Serum sHLA-?(P<0.05).The macrophage counts of patients with COPD grade ?? were significantly negatively correlated with Serum sHLA-? and Serum sHLA-?(P<0.05).There was no significant correlation between eosinophil count and EBC sHLA-?,Serum sHLA-?,EBC sHLA-?,and Serum sHLA-?(P>0.05).(5)The levels of ?L-6,?L-8,CRP,?NF-? and TNF-? in COPD group were higher than those in healthy control group(P<0.05).The levels of ?L-6,?L-8,CRP,?NF-? and TNF-? in patients with COPD increased gradually with the severity of lung disease(P<0.05).?L-6,?L-8,CRP,?NF-? were positively correlated with EBC sHLA-?,Serum sHLA-?,EBC sHLA-?,and Serum sHLA-? in patients with COPD class ? and COPD class ??(P <0.05);There was a positive correlation between TNF-? and Serum sHLA-? in patients with COPD class ?(P<0.05).There was a significant positivecorrelation between TNF-? and EBC sHLA-?,Serum sHLA-?,EBC sHLA-? and Serum sHLA-? in patients with COPD grade ??(P <0.05).Conclusions (1)EBC sHLA-?,Serum sHLA-?,EBC sHLA-? and Serum sHLA-? in patients with COPD are higher than those in healthy people,and gradually increase with the progression of the disease.(2)EBC sHLA-?,Serum sHLA-?,EBC sHLA-? and Serum sHLA-? levels were significantly associated with pulmonary function,inflammatory cell levels and inflammatory response factors in patients with COPD,and sHLA-? and sHLA-? were involved in COPD.
