Objective:To compare the clinical efficacy of retroperitoneal laparoscopic partial nephrectomy and retroperitoneal laparoscopic radical nephrectomy in our hospital for T1 b renal cell carcinoma,and provide some data reference for our hospital and even all clinicians.Methods:Retrospective analysis of 126 cases of T1 b renal cell carcinoma in the Department of Urology,the First Affiliated Hospital of Nanchang University from January 2012 to March 2015,including 64 cases of retroperitoneal laparoscopic radical nephrectomy,retroperitoneal laparoscopic surgery 62 patients with partial nephrectomy.Comparison of postoperative laparoscopic radical nephrectomy and partial laparoscopic partial nephrectomy for T1 b renal cell carcinoma,intraoperative blood loss,blood transfusion rate,postoperative hospitalization complications,postoperative pathology,postoperative hospital stay After the discharge,the indicators of renal function changes and tumor recurrence were reviewed.Results:1.There was no statistical difference in the amount of bleeding,blood transfusion rate,postoperative complication rate,postoperative pathological condition,and postoperative hospital stay in the laparoscopic partial nephrectomy group and the retroperitoneal laparoscopic radical nephrectomy group.The significance of learning(P>0.05);the difference of operation time between the two methods was statistically significant(P<0.05).2.After laparoscopic radical nephrectomy,the serum creatinine level was reviewed at 3 months,6 months,and 12 months after surgery.Paired t-test,including 3 months and 6 months,the serum creatinine and preoperative serum creatinine were compared.The difference was not statistically significant(P>0.05).The 12 th and 36 th months after operation were compared with preoperative serum creatinine.After paired t-test,the difference was statistically significant(P<0.05).In the retroperitoneal laparoscopic partial nephrectomy group,the changes of serum creatinine were not obvious in the third month,the sixth month,the 12 th month and the 36 th month after surgery.The difference was not statistically significant.Significance(P>0.05),and there was no significant difference in RI between the 3rd,6th and 12 th months after operation(P>0.05).3.During the follow-up period,3 patients in the retroperitoneal laparoscopic radical nephrectomy group had recurrence,the recurrence rate was 4.69%,and 2 patients in the retroperitoneal laparoscopic partial nephrectomy group had tumor recurrence with a recurrence rate of 3.23%.There was no significant difference in recurrence rate(P>0.05).Conclusion:Post-laparoscopic partial nephrectomy for T1 b renal cell carcinoma has similar tumor control rate and postoperative complications after retroperitoneal laparoscopic radical nephrectomy.However,it can better preserve kidney function,Therefore,we believe that retroperitoneal laparoscopic partial nephrectomy is superior to retroperitoneal laparoscopic radical nephrectomy when conditions permit.
