| ObjectiveThe purpose of this study was to investigate the level of post-traumatic growth of patients undergoing radical surgery for gastric cancer,to analyze its correlation with quality of life,and to investigate and explore the influencing factors and promotion strategies of post-traumatic growth of patients with gastric cancer from the perspective of positive psychology.To provide theoretical and empirical evidence for the development of relevant intervention measures in clinical practice.MethodsA convenient sampling method was used to select the inpatients or outpatients after radical resection of gastric cancer who came from a three-A cancer hospital in November 2017 to November 2018.The Post-traumatic growth scale and the third edition of the Chinese version of EORTC QLQ-C30 were used to investigate the general situation,post-traumatic growth level and quality of life of patients respectively.During the study,a total of 216 questionnaires were issued,and 6 invalid questionnaires were lost,the effective recovery rate was 97%.The data analysis used spss22.0 software to describe the results of the questionnaire through the mean,standard deviation,rate,percentage and other statistical indicators.In univariate analysis,independent sample t-test or ANOVA was used for comparison between groups;multivariate stepwise regression analysis was used for the main influencing factors of post-traumatic growth level;and partial correlation analysis was used for correlation analysis.Results1.Post-traumatic growth of patients undergoing radical resection of gastric cancerThe total score of post-traumatic growth was(63.22 ±10.51),which was at a moderate level.The dimension score of enjoy life was the highest(3.21±0.55),the second is the score of relationship with others(3.20±0.65),the score of new possibility(3.14±0.61),the score of mental change(3.08±0.52),the score of personal strength(2.77±0.48).2.Univariate analysis of post-traumatic growth in patients with gastric cancer undergoing radical resectionIn univariate analysis,the scores of posttraumatic growth(65.06±19.09)in patients under 45 years old were lower than those in the other two groups(65.28±19.00)and(71.23 ±10.98).There was a significant difference between < 45-year-old group and over-60-year-old group(p<0.05).The scores of posttraumatic growth in married patients(68.41±17.19)were higher than those in unmarried patients(60.09±11.82)and divorced / widowed patients(62.97±10.35)(p<0.05)..There were significant differences in post-traumatic growth of patients with different monthly family income(p<0.01),post-traumatic growth of patients with different educational levels(p<0.05),and post-traumatic growth of patients with different family income(p< 0.05).There was a significant difference in post-traumatic growth among patients with different medical payment methods(p<0.01),and there was significant difference in post-traumatic growth among patients with different personality characteristics(p<0.05).There was significant difference in post-traumatic growth among patients with different postoperative time(p<0.01),and there was significant difference in post-traumatic growth among patients with different surgical methods(p<0.05).3.Univariate analysis of influencing factors of post-traumatic growth in patients with gastric cancer undergoing radical gastrectomyMultivariate regression analysis showed that: age,postoperative time,medical payment and personality type can explaine 67.2% of the total variation.4.Current status of quality of life in patients with gastric cancer after radical resectionThe quality of life score of 210 patients after radical gastrectomy was(52.90±20.93),and the score of social function was the highest in 5 functional areas,followed by cognitive function,role,emotion,and physical function,and the score of social function was the highest in the 5 functional areas,followed by the lowest score in cognitive function,role,emotion and physical function.In the symptom field,fatigue,nausea and vomiting scored the highest in latitude;other dimensions of insomnia,constipation,and economic difficulties scored higher.5.Correlation between post traumatic growth and quality of life in patients undergoing radical operation of gastric cancerPearson correlation analysis showed that there was a positive correlation between post-traumatic growth and quality of life(r=0.642,p<0.01).The total score of post-traumatic growth was positively correlated with the overall quality of life score(r = 0.642,0.636,0.572,0.507,0.571,0.267,p<0.01).Conclusions1.After radical resection of gastric cancer,the patients have post-traumatic growth,which is at the middle level.The dimension of appreciation of life is perfect,followed by relationships with others,new possibilities,mental changes and personal strength.Therefore,medical staff should pay attention to improving the level of post-traumatic growth of patients..2.The age,postoperative time,medical payment,education level and personality type of patients undergoing radical operation of gastric cancer were the main influencing factors of PTG.3.After radical resection of gastric cancer,the quality of life of the patients is at a low level,and the five functional areas are not high.The patients are prone to fatigue,nausea and vomiting,insomnia,constipation and financial difficulties in the course of treatment..4.The posttraumatic growth of patients after radical resection of gastric cancer is positively related to their overall quality of life.Therefore,guiding the occurrence of post-traumatic growth of patients is beneficial to improve the quality of life. |
