| Background: Gastric cancer(GC)is the fifth most common carcinoma and the third leading cause of cancer related death worldwide,and is the second most common cancer and third leading cause of cancer death in China.The development of GC is caused by different factors.Tumor suppressor gene p53(TP53)mutation and Cadherin-1(CDH1)mutation are corresponded with the prognosis of GC,which are related to the prognosis of GC.Purpose: To analyze the relationship between TP53/CDH1 mutation and clinicopathological features and prognosis of gastric cancer,and to analyze the relationship between clinicopathological features and prognosis of gastric cancer.Methods: The patients with gastric cancer were treated in the Department of Gastrointestinal and Gland Surgery,First Affiliated Hospital of Guangxi Medical University from September 2017 to December 2018.Gastric cancer tissue samples from these patients were collected for TP53 and CDH1 mutation detection.We collected the clinicopathological features of GC,including gender,age,tumor location,tumor size,histological differentiation,vascular invasion,perineural invasion,lymph node metastasis,TNM stage and serum CEA,et.al,and all the patients were conducted with follow-up observation.The relationship between TP53/CDH1 mutation and clinicopathological features and prognosis of gastric cancer were analyzed with statistical methods.Results: 1.There were 38 cases who met the inclusion criteria concluded in the article,including 23 males and 15 females.The range of age is from 25-79 years old,the median age is 55.2.19 cases of TP53mutation(50.0%)and 10 cases of CDH1 mutation(26.3%)and 2(5.3%)cases of both of mutation were showed in the article respectively.The majority mutation of both of the two genes were missense mutation and deletation.TP53 mutation was correlated with serum Carbohydrate antigen 19-9(CA19-9)(P=0.036),the rate of TP53 mutation in high level and normal CA19-9 were 85.7% and 41.9% respectively.CDH1 mutation was correlated with tumor differentiation(P=0.038),whose mutant rate in poor differentiation and well differentiation were 35.7% and 0.0% respectively.3.All patients were followed up until March 31,2019.There were 5 cases(13.2%)of loss of follow-up,2 cases(5.2%)of metastasis/recurrence and 6 cases(13.2%)of death including in the article.1-year’ disease-free survival(DFS)and overall survival(OS)were 73.3% and 80.0% respectively.No significant relationship between TP53 mutation and CDH1 mutation with the DFS and OS of GC(all p>0.05).4.The results of univariate analysis presented that DFS and OS of gastric cancer patients were correlated with tumor location and serum CEA level(p<0.05).COX multivariate analysis showed that tumor location was an independent risk factor for DFS(p=0.029)and serum CEA expression level was an independent risk factor for OS in patients(p=0.008).Conclusion: 1.TP53 mutation is significantly correlated with serum CA19-9,whose mutant rate in high level group is higher than that in normal group.2.CDH1 mutation is correlated with tumor differentiation,whose mutant rate in poor-differentiated group is higher than that in well-differentiated group.3.The DFS of upper gastric cancer is worse than that in middle and lower cancer.4.Patients with elevated serum CEA has worse OS than those with normal levels. |
