Objective: To explore the relationship between outpatient male sperm DNA fragmentation index(DFI)and the basic condition and semen parameters of infertile men,and the relationship between DFI and fertilization,embryo quality and clinical outcome of in vitro fertilization-embryo transfer.Method: Collecting data of IVF-ET and its frozen-thawed and embryo transfer cyclics.Sperm chromatin structure assay was used to analyse outpatient male sperm DFI.Divide three group according to DFI level(A:DFI <15%,B:15%?DFI?30%,C:DFI > 30%).The relationship between semen parameters,embryonic development and clinical outcomes was compared between groups.Result: There were no significant differences in the infertility years and body mass index(BMI)between the three groups(P>0.05).The age of the women in the C group was higher than the other two groups,the difference was statistically significant(P<0.05).Significant correlation of sperm DFI with male age,the percentage of forword-moving sperm,normal sperm-morphology,semen volume were observed(P<0.01).It was signally correlated with the fertilization rates and Normal fertilization rates between the three groups(P<0.01).Therewere no significant differences in the indexes of embryo development observed between the groups(P>0.05).The clinical pregnancy outcome of IVF-ET was not statistically significant(P>0.05),but with the increase of sperm DFI,the biochemical pregnancy rate and live birth rate decreased.The clinical outcome of the FET cycle,there was no significant difference between the different DFI groups,but the abortion rate of group A to group C gradually increased,and the cumulative pregnancy rate and cumulative live birth rate decreased.Conclusion : DFI is one of the index to assess male fecundity,DFI will increase with the increase of male age,which will reduce the sperm quality and the fertilization ability of sperm.In the analysis of the IVF-ET and its FET cycles,sperm DFI has not significantly correlated with embryo quality and clinical outcome,however,outpatient male sperm DFI may have predictive effect.
