| Objective To investigate the relationship between serum vitamin B12 level and sudomotor function in patients with type 2 diabetes mellitus.Methods A total of 501 patients with T2DM who were admitted to our department were enrolled.The conductivity analyzer SUDOSCAN was used to measure the skin electrical conductivity(ESC)as an index for detecting sudomotor function(SF).The mean value of both hands(HESC)and the mean value of feet conductance(FESC)were>60μs for 326 patients with normal sudomotor function(NSF group),and HESC or FESC<60μs for 175 patients with abnormal sudomotor function(ASF group).comparing the clinical biochemical indicators between the two groups and the results of SUDOSCAN detection.Results(1)The age,course of disease,creatinine level,hand mean asymmetry(HASYM)and foot mean asymmetry(FASYM)in abnormal sudomotor function group were significantly higher than those in normal sudomotor function group,while the levels of HESC,FESC and vitamin B12 were lower than those in normal sudomotor function group[(51.0114.5)vs(75.1±7.5)μs,(52118.4)vs(77.4±7.6)μs,(518±193)vs(593±212)pg/ml,P<0.01].(2)Compared with the normal sudomotor function group,the rate of vitamin B12 deficiency was not significantly different(P>0.05),and the rate of marginal depletion was higher than the normal sudomotor function group(x2 =9.72,P<0.01).There was no significant difference in vitamin B12 levels,the rate of vitamin B12 deficiency and the rate of marginal depletion between male and female groups(P>0.05).(3)Spearman correlation analysis showed that HESC and FESC were positively correlated with VB12 level,negatively correlated with creatinine and disease course(P<0.05),HASYM and FASYM were positively correlated with disease duration(r=0.15,r=0.15,P<0.05),FASYM was negatively correlated with LDL-C(r=-0.1,P<0.05).(4)Vitamin B12 levels had effects on HESC and FESC levels(F=9.19,F=10.4,P<0.001),and had no significant effect on HASYM and FASYM(P>0.05).(5)Multivariate stepwise linear regression analysis showed that the course of disease was an independent risk factor for HESC(β=-0.031,P<0.01).The course of disease and creatinine were independent risk factors for FESC(β=-0.036,,β=-0.07,P<0.05).Vitamin B12 is an independent protective factor for HESC and FESC(β=0.027,β=0.017,P<0.01).Conclusion The level of serum vitamin B12 in patients with type 2 diabetes had an effect on the levels of HESC and FESC,and was positively correlated with the levels of HESC and FESC. |
