Optimum Selection Of Electro-acupuncture Analgesic Treatment For Bone Cancer Pain In Rats Based On Orthogonal Design Experiment

Objective: To study the analgesic effect of different combination therapies consists of the three levels of four factors(Different acupoint groups: Zhongwan-Neiguan,Hegu-Taichong,Zusanli-Sanyinjiao;electro-acupuncture time: 30 minutes,45 minutes,60 minutes;electro-acupuncture frequency: fixed low-frequency 2HZ,fixed high-frequency 100 HZ and dense wave 2/100HZ;current size: 1m A,2m A,1/2m A)on bone cancer pain in rats,and to optimize the best treatment of bone cancer pain in rats.Method: Seventy clean-grade male SD rats weighing 190-230 g were screened and domesticated in synchronous light-dark cycle for 7 days under the condition of L:D 12:12 in the laboratory.66 rats with good adaptability to light-dark cycle were retained.The basic pain threshold of the retained rats was screened by tail flick method at 7-12 a.m.the day before the start of the formal experiment.The rats with high pain threshold(latency of tail flick reaction > 10s)and hyperalgesia(< 3s)were excluded.Sixty-three rats which met the inclusion criteria of this experiment were selected and randomly divided into nine groups according to the principle of complete randomization by SPSS23.0 statistical software.On the first day of the experiment,three SD healthy rats were randomly selected from each group for X-ray observation of the left hind limb.Then Walker 256 breast cancer cells were inoculated into the proximal intercondylar fossa of the left hind limb of the rats,and the skin of the operation site was disinfected with iodine complexation.The rats were put back into the cage after recovering from anesthesia.The rats’ life activities were carefully observed the next day after operation.The left hind limb walking,body general condition,life rhythm and tumor growth of rats inoculated with bone cancer cells were observed every other day.The growth of bone tumors in rats was examined with the microphotographic imaging system moticam3000.On the 12 th day of the experiment,one rat was randomly selected from each group and its left posterior femur was taken for pathological examination.The pain threshold of rats was measured by tail flick of thermal radiation light on the 2nd day after the successful operation(before electro-acupuncture treatment).The rats in each group were treated by combination therapy based on four factors multiplied by three levels(different acupoint groups: Zhongwan-Neiguan,Hegu-Taichong,Zusanli-Sanyinjiao;electro-acupuncture time: 30 minutes,45 minutes,60 minutes;electro-acupuncture frequency: fixed low-frequency 2HZ,fixed high-frequency 100 HZ and dense wave 2/100HZ;current size: 1m A,2m A,1/2m A).At the end of EA treatment in each group,the pain threshold was measured again with the same method and recorded by another person.The experimental data were analyzed by statistical software SPSS23.0.The rats were given electro-acupuncture analgesia for 6 days as a course of treatment.After 2 courses of treatment,the pain threshold of rats was measured again.The rats were not treated for 1 day between the two courses.The data of body weight and pain threshold were expressed as mean±standard deviation(m±s).After normal distribution test and homogeneity test of variance,the variance analysis of repeated measurements was used for comparison among groups.P < 0.05 showed that there was a significant difference between the two groups.The elevated pain threshold of healthy SD rats was taken as the final evaluation index of therapeutic effect.Visual analysis,variance analysis and multiple comparisons of the experimental data were made.The best acupoint selection group,frequency,time and current intensity of electro-acupuncture for treating cancer pain were determined preliminarily,thus to establish the optimal treatment for pain relief of bone cancer by electro-acupuncture.Results: There was no significant difference in body weight and basic pain threshold among SD healthy rats(P > 0.05),so it was comparable.The pain threshold phase of rats changed after the successful establishment of bone cancer pain model,and the proliferation of rat femoral cancer cells significantly affected the pain threshold of rats in each group(all P < 0.05).In terms of the immediate analgesic effect of electro-acupuncture analgesia,through analysis of variance we know that factors such as acupoint group(A),frequency(B),time(C)and current(D)were significant factors.By multiple comparisons among levels of various factors we can know that among the factors of A(Acupuncture Point Group),the analgesic effect of A3(Hegu-Taichong)level was significantly better than that of the other two levels,among B(electro-acupuncture frequency)factors,the analgesic effect of B3(2/100HZ)level was better than that of the other two levels,the analgesic effect of C1(30 min)level was better than that of the other two levels in C(retention time),among the D(current magnitude)factors,the analgesic effect of D2(1/2m A)level was significantly better than that of the other two levels.In the respect of Long-term analgesic effect of electro-acupuncture analgesia,we know from the analysis of variance that both Acupuncture Point Group(A)and Needle Retention Time(C)were significant factors.Through multiple comparisons among factors and levels we found that the analgesic effect of A3(Hegu-Taichong)level was better than that of the other two levels(P < 0.01),the analgesic effect of C1(30 min)level was better than that of C3(60 min)level in C factor(P < 0.05).Conclusion: The effect of electro-acupuncture analgesia on cancer pain is remarkable.In this experiment,four factors of electro-acupuncture analgesia for cancer pain have significant influence on the effect of electro-acupuncture analgesia.the best combination of various factors in the treatment of cancer pain by electro-acupuncture analgesia was A3B3C1D2.The optimal scheme of electro-acupuncture analgesia for cancer pain was that high and low frequency conversion(2/100HZ)electro-acupuncture acted on Hegu-Taichong acupoint group for 30 minutes,and the current of 1 m A lasted 15 minutes,and that of 2 m A lasted 15 minutes.In the long-term efficacy evaluation,among the four factors of electro-acupuncture analgesia for cancer pain in the design of this experiment.Acupuncture acupoint group(A)and electro-acupuncture time(C)had significant effects on electro-acupuncture analgesia.The best combination of the factors of EA analgesia for cancer pain was A3B3C1D1.It means that the optimal scheme of electro-acupuncture analgesia for cancer pain is that high and low frequency conversion(2/100HZ)electro-acupuncture acts on Hegu-Taichong acupoint group,and electro-acupuncture with 1 m A current for 30 minutes continuously.
