ObjectiveTo explore the incidence of abnormal bone mineral density(BMD)in young and middle-aged patients with ankylosing spondylitis(AS)and the correlation of commonly used clinical indicators.MethodsFrom July 2014 to August 2018,hospitalized patients admitted to Rheumatology and Immunology Department of Shanghai Changzheng Hospital due to AS and health examinees from the Health Examination Center of Shanghai Changzheng Hospital were collected,all of them were younger than 50 years old.Bone mineral density of lumbar spine and femoral neck in AS patients and healthy control group was measured by dual energy X-ray.BMI,drug use,ESR,CRP,platelet volume,platelet count,serum uric acid,AKP and calcium ions were compared and analyzed with age,course of disease,iritis,smoking,sex,height,weight,BMI,drug use.Results(1)A total of 47 cases of abnormal bone mass were detected in 193 AS patients,with an incidence of 24.35%.There was no statistical difference in the incidence of abnormal bone mass among the three groups.(2)BMD in all parts of AS patients was lower than that in healthy control group(P < 0.05).(3)BMD was correlated with serum calcium and CRP in AS patients(P < 0.05),but not with age,platelet volume,platelet count,ESR,serum uric acid,AKP,height,weight and BMI.(4)The incidence of abnormal bone mass in AS patients was correlated with sex(P < 0.05),but not with drugs,iritis and smoking.(5)BMD of lumbar spine in AS patients was not linear with course of disease,but BMD of femoral neck was linear with course of disease(P < 0.05).ConclusionThe incidence of abnormal bone mass in AS patients is higher than that in normal people of the same age.The factors related to BMD in AS patients may be gender,CRP and blood calcium,so whether controlling inflammation and supplementing calcium can prevent abnormal bone mass needs further study.BMD in femoral neck of AS patients decreases with the course of disease,but BMD in lumbar spine has nothing to do with the course of disease,considering whether it is related to the surrounding group of lumbar spine.Tissue ossification is related,so femoral neck BMD may be more helpful than lumbar BMD in the diagnosis of bone mass abnormalities in AS patients.
