BackgroundDental caries is a kind of chronic progressive destruction disease mainly caused by bacteria.The formation of caries is not a simple demineralization process,but a result of alternate demineralization and remineralization.When the demineralization is bigger than the remineralization,the minerals will be lost gradually,and finally the cavity will be formed.Demineralization of early caries is reversible,and the environment that promotes the remineralization of early caries needs to be strengthened.Fluoride is one of the important methods to promote the remineralization of early enamel caries,but the fluoride has some limitations.Many scholars have studied many non-fluorinated remineralization preparations,but at present all the non-fluorinated remineralized preparations have a weaker effect than fluorides.In recent years,some scholars have studied the combination and use of other remineralization agents with fluoride to see if they have synergistic effect,in order to obtain better effect of remineralization to reduce the dosage of fluoride.Our previous in vitro studies have found that the recombinant amelogenin polypeptide can promote early enamel caries to complete the remineralization,but the effect of remineralization was weaker than that of fluoride.ObjectivesBased on the previous study,the synergistic effect of recombinant amelogenin polypeptide and fluoride on early enamel caries remineralization was preliminarily studied in order to provide experimental basis for the development of a safe,effective and effective anti-caries preparation which can promote the early caries remineralization.MethodsWe chose the fresh-extracted bovine maxillary incisor and cleaned the surface.The hard tissue cutting machine was used to make the enamel block with the window size of 4 mm(length)and 3 mm(width).Then,the lip side of enamel was polished by different number of silicon carbide sand paper in turns.40 specimens of artificial enamel caries were prepared in demineralized solution and randomly divided into 4 corresponding remineralized solution groups(10 / group):Group A,fluoridated remineralizing solution(2 ppm NaF)treatment group;Group B,the remineralizing medium solution treatment group;Group C,the remineralizing solution treatment group containing 100?m/ mL of recombinant amelogenin polypeptide;Group D,containing 100?m/ mL of amelogenin polypeptide and fluorinated remineralizing solution(2 ppm NaF)treatment group.The remineralized solution of different groups was remineralized for 12 days and the corresponding fresh solution was replaced every day.After the remineralization process was completed,the amount of mineral loss and the depth of the lesion before and after the remineralization of each of the bovine enamel caries samples of each group was measured by the Transverse Microradiography(TMR).Mean mineral gain after remineralization and the reduction in depth of caries can also be obtained in each specimen.ResultsBy comparing the mineral loss of each treatment group after demineralization and remineralization,the mineral loss and caries depth of each treatment group decreased after remineralization(P<0.05).The fluorinated remineralized solution treatment group,the remineralized solution containing 100 ? g / mL recombinant amelogenin polypeptide and the fluorinated remineralization solution(2ppm NaF)containing 100 ? g / mL recombinant amelogenin polypeptide compared with the remineralizing medium treatment group obtained relatively higher mineral gain(P < 0 05).At the same time,the depth of caries has also been greatly reduced.There was a significant difference between the recombinant amelogenin poplypeptide(100 ? g / mL)fluorination(2ppm NaF)solution group and the recombinant amelogenin poplypeptide(100 ? g / mL)group(P < 0.05).At the same time,the recombinant amelogenin poplypeptide(100 ? g / mL)fluorination(2ppm NaF)solution obtained a relatively higher mineral yield compared with the 2ppm NaF solution group,but there was no significant difference between the two groups(P > 0 05).ConclusionThis experiment preliminarily confirmed that the combination of recombinant amelogenin poplypeptide and fluoride had no synergistic effect on the early enamel caries remineralization.
