A Preliminary Study Of Resting State Functional Magnetic Resonance Imaging In Patients With Cognitive Impairment Of Parkinson’s Disease

Abstract/Summary:
Objective: By applying the low-frequency amplitude method(ALFF)and Regional Homogeneity(ReHo)of the resting state functional magnetic resonance(RS-fMRI)and analyzing the changes of resting brain function in Parkinson’s disease patients with cognitive impairment,we aim to investigate the neural activity characteristics and offer some help for the imaging diagnosis of Parkinson’s patients with cognitive impairment.Methods: We collected 32 Parkinson’s disease(PD)patients diagnosed in the department of neurology of the Affiliated Hospital of Guizhou medical university and 16 healthy controls(HC)without cognitive impairment with matching gender,age and years of education from December 2017 to December 2018.All the PD patients were assessed by the Montreal Cognitive Assessment scale(MoCA),so the PD patients were further divided into two groups: Parkinson’s disease with cognitive impairment(PD-CI)and Parkinson’s disease with normal cognitive function(PD-NC).We collected the resting state functional magnetic resonance(RS-fMRI)data of the subjects,and obtained the brain regions with statistical differences by using REST software and ALFF 、ReHo algorithm.Results: 1.When the PD-CI group compared with the HC group,the ALFF values of bilateral medial occipital lobe,right calcarine,left cerebellum45,left cerebellum6,left fusiform gyrus,bilateral precuneus and left cuneus decreased in PD-CI group;the increased ALFF values were found in right middle frontal gyrus,left medial superior frontal gyrus,left inferior frontal gyrus triangle,left cerebellum8.The ReHo values were decreased in Bilateral inferior frontal gyrus,left middle frontal gyrus,bilateral superior temporal gyrus,right middle temporal gyrus,left parietal lobule,right parietal gyrus,right superior marginal gyrus,cingulate gyrus,right peritalar fissure cortex,right cerebellum1;the ReHo values were increased in left cerebellum 2,right cerebellum 8,bilateral precuneus,corpus callosum,right dorsolateral superior frontal gyrus.2.When the PD-CI group compared with the PD-NC group,the ALFF values of the right superior temporal gyrus,right inferior parietal lobule and right superior parietal lobule were decreased in the PD-CI group;the ALFF value were increased in left frontal gyrus rectus,right frontal gyrus rectus,right superior frontal orbit and bilateral supplementary motor area in the PD-CI group.The ReHo values were decreased in bilateral frontal lobe,cingulate gyrus,right thalamus,right caudate nucleus,right calcarine,left cerebellum6 and left fusiform gyrus in the PD-CI group;the ReHo values of left frontal lobe and bilateral cerebellar were increased in the PD-CI group.3.When the PD-NC group compared with the HC group,the ALFF values in left frontal gyrus rectus,left superior frontal orbit,left external capsule area,left hippocampus and bilateral supplementary motor area were decreased in the PD-NC group;the ALFF values in left medial superior frontal gyrus,left superior frontal orbital gyrus,bilateral precuneus were increased in the PD-NC group.The ReHo values of the left frontal lobe and the left putamen were decreased in the PD-NC group;The ReHo values of bilateral cerebellum,bilateral thalamus,left caudate nucleus,left frontal lobe and cingulate gyrus were increased in the PD-NC group.Conclusions: Decreased activity in part of the frontal lobe,parietal lobe,temporal lobe and cingulate gyrus,right thalamus,right caudate nucleus is associated with cognitive impairment in PD patients.Increased activity in frontal lobe,cerebellum and other brain areas may be the manifestation of cognitive compensation.RS – fMRI can find the brain’s functional changes in the early stage of lesion,this may provide a new imaging method for the early identifacation of PD-CI patients.
