Purpose:To discuss and analyse the clinical outcomes of buttress plate for posterior column fractures of tibial plateau,review experiences and further elevate clinical effects so as to make this technique for more extensive use.Methods:Prospectively evaluate thirty patients who suffer from posterior column fractures of tibial plateau from January 2017 to January 2018.All of them are closed,single,clearly knee injuries(exclude patients with complications,such as brain heart,lung,liver,kidney,hematologic,immunological diseases and psychopath)adopt buttress plates for posterior column fractures.Among them,there were 21 males and 9 females,age ranges from 20 years to 70 years,mean age 46.4 years.Postoperative follow-up ranges from 14 months to 26 months,mean follow-up is 17.1 months.Every case proceed adequate preoperative preparation.Record length of surgery,amount of bleeding,length of hospitalization,postoperative complication,fracture healing and full-bearing time.TPA and PSA were measured immediately after surgery and fourteen months after surgery and functional outcomes based on Hospital for Special Surgery and SF-36 were also recorded.Results:All the cases applied buttress plate for posterior column fractures were followed for average 17.1 months(range,14-26 months).The average operative time and mean blood lose was 138.2 minutes and 343.3ml.The average radiographic bony union time and the average full weightbearing time were 13.1 weeks and 16.7 weeks.The mean Short Form 36,Hospital for Special Surgery and low extremity measure score at 14 months postoperatively were 90.2(range,90.2±4.1),89.2(range,89.2±4.2)and 87.2(range,87.2±3.9).The average range of motion of the affected knee was 2.7° to 123.6° at 14 months after the operation.There were no significant differences in either TPA or PSA.Conclusion:Back buttress plate for posterior column fractures is a reliable method which can promote early knee functional exercise especially for complicated tibial plateau fracture.
