ObjectiveTo determine specific characteristics of contrast-enhanced ultrasonography(CEUS)of thyroid papillary carcinoma and explore the value of CEUS in the diagnosis of thyroid papillary carcinoma using a Logistic regression model.Explore the correspondence between ultrasound imaging features and pathology.Methods119 patients who underwent CEUS in our hospital from October 2016 to December 2018 were retrospectively analyzed.Eight variables were included in the logistic regression model: peak time,enhancement uniformity,presence of ring enhancement,change in nodule size after enhancement,presence of perfusion defect,nodule beginning enhancement area,peak intensity,nodule boundary after enhancement.Logistic regression model was established with pathological results as the gold standard.The ROC curve was drawn to evaluate the predictive ability of the Logistic regression model,and the sensitivity and specificity of CEUS in the diagnosis of thyroid papillary carcinoma were evaluated by comparing the likelihood ratio of each variable.The selected contrast-enhanced features were compared to the pathology.ResultsThe characteristic variables of CEUS with statistical differences include ring enhancement,low enhancement,inhomogeneous enhancement,and perfusion defect.Finally,there are three characteristic variables of CEUS included in the Logistic regression model: low enhancement,perfusion defect,and inhomogeneous enhancement.The accuracy of Logistic regression model in predicting thyroid papillary carcinoma was 90.5%(114/126),the sensitivity was 94.2%,and the specificity was 82.5%.Central perfusion defect has higher diagnostic specificity than simple perfusion defect.ConclusionsContrast-enhanced ultrasonography is of high diagnostic value in the diagnosis of thyroid papillary carcinoma,among which "low enhancement,perfusion defect and inhomogeneous enhancement" are the contrast-enhanced characteristics of thyroid papillary carcinoma.The appearance of the three features suggests that the nodule is highly likely to be malignant,and the more likely these features are,the more likely the nodular malignancy is.Perfusion defects have the highest sensitivity for identifying benign and malignant thyroid nodules,but the specificity of peritoneal malignancy by differential diagnosis of thyroid nodules is low.The use of central perfusion defects instead of simple perfusion defects is beneficial to improve the differential diagnosis of perfusion defects.The specificity of benign and malignant thyroid nodules.Therefore,the application of contrast-enhanced ultrasound can further improve the accuracy of ultrasound diagnosis of papillary thyroid carcinoma.
