| Objective:A retrospective study was used to explore the prognostic factors of patients with temporal lobe epilepsy after surgical treatment and to evaluate the curative effect by surgical treatment.Methods:Patients with temporal lobe epilepsy after surgical treatment at the Second Affiliated Hospital of Nanchang University from January 2015 to January 2019 were chosen to analyze the prognostic factors.The evaluation included imaging,scalp EEG,clinical symptomology,preoperative diagnosis and surgical treatment,etc.,69 patients with temporal lobe epilepsy were diagnosed before the neurosurgery,and the clinical follow-up was performed a year after surgery.The preoperative imaging,EEG,seizure symptoms and other data were collected from each patient,and the postoperative results were evaluated by Engel’s scale.The patients were divided into satisfactory group(Engel Ⅰ+ Engel Ⅱ)and dissatisfied group(Engel Ⅲ+ Ⅳ).Then,statistically separate methods were used to analyze the effect of each factor on the prognosis of patients alone.P<0.05 was statistically significant.Results:1.By analyzing 69 patients with temporal lobe epilepsy,few patients deserved postoperative complications.2.There were 45 cases of preoperative MRI with clear imaging changes and 24 cases without clear imaging results.The satisfaction rate of one-year follow-up was 95.55% and 62.50%,respectively.P < 0.05).3.Through the analysis of the patient’s scalp EEG,49 patients with localization limitation by EEG were found,and the rest were 20 patients with multifocal discharge.The satisfaction rate of one year follow-up was 91.83% and 55.00%,respectively.4.Only through the patient’s clinical symptomatic manifestations can determine the side of the patient’s epileptogenic focus,of which 46 cases can be fixed,29 cases can not be determined,the two-year follow-up satisfaction is 100% and 75.86%,respectively.Conclusion:1.Patients with temporal lobe epilepsy have a good prognosis and are affected by a variety of related factors.2.Patients with pre-operative imaging data have a clear structural manifestation,the effect of post-operative treatment is better than imaging negative.3.It is possible to locate the patients with temporal lobe epilepsy by scalp EEG.4.The patient’s clinical symptomology has typical symptoms,which can directly discriminate the surgical effect of the patient’s head epileptic discharge side. |
