Glioma is a generic term of tumors which originated from glial cells in the central nervous system and neurons,It’s the most common primary intracranial tumor,Accounting for about 40%,The high grade glioma(HGG)accounted for about 50% of glioma,The high grade glioma can cause neurological damage with poor quality life of patients because the tumor growth way is invasive with fast.Rapid recurrence after surgery makes the patient’s survival time greatly shortened,At present,surgery combined with radiotherapy and chemotherapy is the main treatment of high grade glioma.Statistical analysis was performed using SPSS.20 software.Kaplan-Meier analysis was used to analyze survival rate.Univariate analysis was performed on related factors that may affect prognosis,and factors that are significant in univariate analysis were included in multivariate analysis.Objective:Analysis of factors affecting prognosis of patients with high-grade glioma.Method:Using the method of retrospective analysis,Patients who underwent neurosurgery in the first Hospital of Nanchang University from January 1,2010 to December 31,2015 and underwent the first intracranial tumor resection and reported a pathological outcome of HGG.Record the patient’s gender,age,preoperative convulsions,preoperative KPS score,and whether there is increased intracranial pressure(according to whether the patient has clinical manifestations of headache,vomiting),tumor diameter,tumor resection range,postoperative pathological results,and immune group Information and other information.Telephone follow-up patients from the date of surgery from the date of surgery and postoperative radiotherapy and chemotherapy.Result:Survival analysis showed that the 1-,2-,and 3-year survival rates of HGG patients were 60%,23%,and 4%.Univariate analysis showed that age,epilepsy,preoperative KPS score,degree of tumor resection,postoperative pathological grade,radiotherapy and chemotherapy,and Ki-67 expression were the prognostic factors affecting glioma(P<0.05).Multivariate analysis of indicators such as age,epilepsy,and preoperative KPS scores with statistically significant univariate analysis.Using the analyze method of COX regression model.The results showed that the extent of tumor resection,postoperative pathological results,and postoperative chemotherapy were independent factors affecting patient survival(P<0.05).Conclusion:Survival analysis and analysis by single factor and multi-factor methods.The high grade glioma patients average survival time is 16.9 months.The younger the age,the lower the pathological grade,the lower the Ki-67 expression,the occurrence of preoperative epilepsy,the higher the preoperative KPS score,the higher the degree of tumor resection,and the better the prognosis of the patients receiving postoperative radiotherapy and chemotherapy.Among the many influencing factors,the younger the age,the lower the pathological grade,the lower the expression of Ki-67,the preoperative seizure,the higher the preoperative KPS score,the higher the degree of tumor resection,and the better prognosis in patients undergoing radiotherapy and chemotherapy.
