Clinical Analysis Of 57 Cases Of Cervical Insufficiency Treated By Transvaginal Cervical Cerclage

Objective: Retrospective analysis of clinical data of patients with cervical insufficiency,exploration of different surgical timing and surgical methods of transvaginal cervical cerclage on pregnancy outcomes.Methods: From January 2014 to December 2018,57 cases of pregnant women who underwent transvaginal cervical cerclage due to cervical insufficiency were collected,which were divided into prophylactic cervical cerclage group(30 cases),stress cervical cerclage group(11 cases)and emergency cervical cerclage group(16 cases)according to different surgical timing.According to different surgical methods,it was divided into simplified Shirodkar cerclage(41 cases)and McDonald cerclage(16 cases).To compare the differences in the prolonged gestation time,pregnancy weeks,pregnancy outcomes,delivery methods,survived infant weight,Apgar score at 1 minute of birth and complications of transvaginal cervical cerclage in patients with different surgical timing and surgical methods Results: 1.There were differences in prolonged gestation time,gestational weeks,pregnancy outcomes,delivery methods,survived infant weight and complications of transvaginal cervical cerclage at three different surgical timings(P <0.05),there was no significant difference in the success rate,early birth rate and abortion rate,and Apgar score at 1 minute of birth(P >0.05).2.The simplified Shirodkar cerclage and McDonald cerclage of transvaginal cervical cerclage had no difference in prolonged pregnancy time,pregnancy weeks,pregnancy outcome,survived infant weight,Apgar score at 1 minute of birth and complications(P >0.05),there was a difference in the mode of delivery(P <0.05).Conclusion: 1.In the cervical cerclage of three kinds of surgical timing,the prophylactic cervical cerclage group has the best effect,and the emergency cervical cerclage group has relatively poor effect.Therefore,for patients with cervical insufficiency,they should strive to perform the prophylactic cervical cerclage before the change of cervical length and shape.2.Both the simplified Shirodkar cerclage and McDonald cerclage are the alternative suture methods for transvaginal cervical cerclage.
