| Objetive:1.Using the phenomenological research method in qualitative research to study the spiritual care needs of elderly patients with chronic heart failure.2.Evaluate the application effect of spiritual care measures(three-step method of touching and communicating)in elderly patients with chronic heart failure.Method:1.From January to June 2018,a semi-structured interview was conducted among10 elderly patients with chronic heart failure using the purpose sampling method to obtain the theme of spiritual care needs.2.From June to November 2018,70 cases of the subjects who met the inclusion criteria in a third-grade general hospital of Shanxi Province were selected and grouped by random number table method,including 35 cases in the intervention group and 35 in the control group.The intervention group implemented the three-step method of touching and communicating,and the control group implemented routine nursing intervention methods,using the Self-rating Anxiety Scale(SAS),the Newfoundland Memorial University Happiness Scale(MUNSH),blood pressure,and heart rate.Result:1.The theme of spiritual care needs for elderly patients with chronic heart failure:(1)physical and mental comfort,emotional release(2)role change,self-esteem(3)social support,relief of worry(4)spiritual support,death education.2.(1)Comparison between groups before intervention: There was no significant difference in the general data between the intervention group and the control group(P>0.05).There were no significant differences in SAS scores,MUNSH scores,blood pressure and heart rate between the two groups(P>0.05).(2)Post-intervention comparison between groups: After the intervention,the SAS score of the intervention group was lower than that of the control group(P<0.05).After the intervention,the MUNSH score of the intervention group was higher than that of the control group,which was statistically significant(P<0.05);There was no significant difference in blood pressure between the two groups after intervention(P>0.05).There was no significant difference in heart rate between the two groups after intervention(P>0.05).(3)After the intervention,the SAS scores of the intervention group were significantly lower than those before the intervention,the difference was statistically significant(P<0.05),and the SAS score of the control group was not significantly different from that before the intervention(P>0.05);the MUNSH score of the intervention group was significantly higher than that before the intervention,the difference was statistically significant(P<0.05),and the MUNSH score of the control group was lower than that before the intervention.The difference was statistically significant(P<0.05).The blood pressure value of the intervention group decreased compared with that before the intervention.Statistical significance(P<0.05),the blood pressure of the control group decreased compared with that before theintervention,the difference was statistically significant(P<0.05);the heart rate of the intervention group decreased compared with the intervention,the difference was statistically significant(P<0.05),the heart rate of the control group The difference was statistically significant(P<0.05).Conclusion:1.There are four aspects of spiritual care needs in elderly patients with chronic heart failure:(1)physical and mental comfort,emotional release;(2)role change,self-esteem;(3)social support,alleviate worry;(4)spiritual support,death education.2.Touching and communicating three-step method can meet the spiritual care needs of elderly patients with chronic heart failure,can improve the anxiety and improve happiness of elderly patients with chronic heart failure,and it is worth promoting. |
