Objective:To investigate the effect of dexmedetomidine on the quality of recovery in general anesthesia patients during analgesia. Methods:Sixty patients undergoing elective laparoscopic assisted radical gastrectomy were divided into four groups,15 in each group.Group C(blank control group)was given intravenous injection of 5 ml of normal saline 30 min before the end of surgery;group F(flurbiprofen axetil group)was given 50 mg of flurbiprofen axetil 30 min before the end of surgery;group D(Rometo In the midazoin group,0.5ug/kg dexmedetomidine was given intravenously 30 minutes before the end of the operation, and it was dripped within 20 minutes.Group M(small dose of dexmedetomidine combined with flurbiprofen axetil)was given intravenous injection of 50 mg of flurbiprofen at 30 min before the end of the operation, while intravenous infusion of 0.25 ug/kg dexmedetomidine was given.Finish.Record MAP,HR before administration(T0),before extubation(T1),immediately after extubation(T2),5 min after extubation(T3),10 min after extubation(T4),and out of PACU(T5)And SpO2;spontaneous breathing recovery time, blink time, extubation time,PACU retention time;VAS pain score,Ramsay sedation score and agitation score when recording T2 ~T5;central venous blood glucose(Glu),substance P(SP)and cortisol(Cor),adrenaline(E),norepinephrine(NE)concentration at T0,T3,T5.Results:The HR,MAP,VAS pain scores and agitation scores in group C were significantly higher than those in the other three groups(P<0.05 or P<0.01).The MAP changes and VAS pain scores of patients in group D were smaller than those of the other three groups, but the Ramsay sedation score was higher than the other three groups, and the recovery time was significantly longer than the other three groups(P<0.05 or P<0.01);Compared with the M group, the spontaneous breathing recovery time and the blink time were prolonged(P<0.05),but the difference between the extubation time and the PACU retention time was not statistically significant. The MAP and VAS pain scores of group M were lower than those ofgroup F(P<0.05 or P<0.01),the Ramsay sedation score was higher than that of group F(P<0.05),and the serum concentrations of Cor,NE,E and SP of group C were at T5.Both were higher than T0,and significantly higher than D group and M group(P<0.05 or P<0.01).Cor concentration in group D and M group was significantly lower than T0 at T5(P<0.05);There was no statistically significant difference in serum concentration in group F(P>0.05).Conclusion:The use of dexmedetomidine alone for analgesia can effectively improve the quality of recovery in patients with general anesthesia, but can prolong its recovery time. When low-dose dexmedetomidine is combined with flurbiprofen,it can improve the quality of anesthesia recovery without prolonging the extubation and PACU retention time.It is a relatively safe and effective analgesic connection method.
