Object: To observe the effects of hemodynamics and awakening situation during Entropy index monitoring in tracheal extubation under propofol sedation in patients of intracranial aneurysms.Method: 40 intracranial aneurysms patients undergoing elective operation were selected form January 2017 to June 2018 at the First Affiliated Hospital of Bengbu Medical College.All patients were randomly divided into entropy group(group E)and control group(group C),with 20 cases in each group.Group E:In the end of operation propofol was preserved pump-infusing maintaining RE at 85~95until extubation which conform to clinical extubation conditions;Group C:Extubation was removed when patients awaked autonomously and consciously without propofol pump-infusing.The changes of HR,MAP,SpO2,RE and SE during preoperative(T1),postoperative(T2).sputum aspiration(T3).extubation(T4).5 min after extubation(T5).10 min after extubation(T6)were recorded.The time ranges from muscle relaxants stopped to recovery of autonomous respiration,from the end of operation to extubation,from the end of operation to consciousness recovery,from the end of operation to the time of leaving PACU,Meanwhile,The score of OAA/S in 10 minutes after extubation and postoperative cough,restlessness,respiratory depression,nausea and vomiting,extubation memory were performed at the same time point.Result: There was no significant difference in age,weight,sex ratio,ASA and operation time between the two groups;There was no difference in HR,MAP,SpO2,RE and SE between the two groups at points of T1 and T2(P>0.05),The differences in HR,MAP,RE and SE at points of T3,T4,T5 and T6 were statistically significant(P<0.05),while the differences in SpO2 were not statistically significant(P>0.05);Compared with T1,the changes of HR and MAP at points of T3 and T4 in group C were statistically significant(P<0.05),However,the changes of HR and MAP in group E were not statistically significant(P>0.05)when comparing with the points of T3 and T4;The differences of postoperative coughing,restlessness,extubation memory,and tongue fallback between two groups were statistically significant(P<0.05),While,the differences of respiratory depression,nausea and vomiting,Compared with group C,OAA/S grade of group E was higher 10 minutes after extubation,and the difference was statistically significant(P<0.05).However,there were no statistically significant differences between the two groups in the recovery time of spontaneous breathing,extubation time,consciousness recovery time and PACU dwell time after surgery(P>0.05).Conclusion: The Entropy index monitoring in tracheal extubation in patients of intracranial aneurysms under propofol sedation has sable anesthesia recovery,low incidence of adverse reactions,while,the time of extubation was not delayed,which obtain higher comfort and worth to clinical promote.
