ObjectiveTo analyze the association of gene mutations and clinicopathologic features in papillary thyroid carcinoma(PTC),and to explore its influence on prognosis.MethodsThe clinical data of 391 patients with papillary thyroid cancer were retrospectively analyzed.The relationship between gene mutations[BRAF V600E,Telomerase Reverse Transcriptase(TERT),RAS,P53,Phosphatidylinositol Kinases 3CA(PIK3CA)]and clinicopathologic features[age,sex,tumor size,local invasion,lymph node metastasis,American Joint Committee on Cancer(AJCC)stage,AMES risk stratification and MACIS score]was analyzed.ResultsThe mutation rates of BRAF V600E and TERT genes in 391 patients were 73.9%and 1.8%.The mutation rates of P53 gene in 135 patients were 1.5%,and the mutation positive rates of RAS family and PIK3CA gene in 180 patients were 2.8%and 0%.The patients with BRAF V600E gene mutation had a larger diameter(0.92+0.58 vs 0.77+0.62,P<0.05)and a higher proportion of local invasion(19.0%vs 10.8%,P<0.05).There were no significant differences in age,sex,complications,multiple diseases and lymph node metastasis.The patients with TERT gene mutation were older than those with TERT gene mutation(53+15 vs 43+13,P<0.05).The proportion of local invasion was also higher(57.1%vs 16.1%,P<0.05).The stage of tumors was relatively late(P<0.05).The risk stratification of AMES and MACIS were relatively high(P<0.05).In addition,there were no obvious differences in factors such as sex,complications,size of tumors,multiple occurrences and lymph node metastasis.ConclusionThe mutation rate of BRAF V600E gene is high,but other genes such as TERT and RAS is low.BRAF V600E and TERT gene mutations may indicate the high invasiveness and high risk of recurrence of papillary thyroid carcinoma,which can provide a reference for recurrence risk assessment and death risk stratification of papillary thyroid carcinoma.
