Study On The Epidemiological Characteristics And Prevention Strategies Of Cerebrovascular And Cardiovascular Diseases In Suzhou

Abstract/Summary:
Currently,the cerebrovascular and cardiovascular disease(CCVD)is an important public health problem and serious threatens to the health in China.In the present study,we firstly analyzed the epidemiological characteristics and the effects of prevention measure on CCVD from 2000 to 2017 in Suzhou.In order to provide the scientific suggestions on prevention measures in the future,we further evaluated the prevention measures on CCVD in the past 10 years using Analysis Hierarchy Process(AHP)in Suzhou.Thus,the present study is consist of two parts,one is "the study on the epidemic situation of CCVD in Suzhou",and the other is "the study on prevention measure of CCVD in Suzhou".Part 1:Study on the epidemic characteristics of CCVD in SuzhouObjectiveTo understand the epidemic situation of CCVD,we analyzed the trend and characteristics of morbidity,mortality and the premature mortality of CCVD from 2000 to 2017 in Suzhou,and analyzed the relationship between CCVD and the life expectancy.Method1.The existing historical CCVD data,including the data of the mortality,morbidity and life expectancy were collected,checked and collated in Suzhou since 2000.2.The death database of relevant years was used to calculate premature mortality,which was obtained by multiplying the death probability of 30-69 years old(5 years as an age group).3.The database of CCVD survey in Suzhou in 2000,2010 and 2015 were collected and collated to calculate the prevalence rate and co-morbidity index.4.To use SPSS(21.0)software to build the database and make statistical analysis.All tests were carried out bilaterally.The significance level of the test was defined as P<0.05.Results1.In 2017,the mortality rate of cerebrovascular diseases in Suzhou was 138.80/100,000,accounting for 19.41%of all deaths(ranking second).Additionally,stroke accounted for 71.63%of cerebrovascular deaths.The heart disease death rate was 81.21/100,000,accounting for 11.36%of all deaths(ranking fourth).The coronary heart disease death accounted for 47.60%of cardiovascular deaths.2.Compared with the incidence and the fatality rate of stroke and coronary heart disease 2010,the incidence of stroke and coronary heart disease increased 1.47 and 1.62 times in 2017.However,the fatality rate of stroke decreased by 35.48%and coronary heart disease increased by 31.11%.3.Compared the fatality rate of cerebrovascular and cardiovascular diseases in 2010,the fatality rate of cerebrovascular diseases decreased by 0.69%in 2017,while the fatality rate of cardiovascular diseases increased by 30.70%:in 2017.The proportion of death caused by stroke decreased from 79.43%to 51.61%in cerebrovascular diseases caused deaths.Additionally,the proportion of death caused by coronary heart disease to the cardiovascular diseases caused deaths fluctuated between 47.45%and 54.87%from 2010 to 2017.4.Since 2008,the premature mortality of cerebrovascular disease,stroke and coronary heart disease has gradually decreased by 47.59%,59.70%and 92.53%,respectively,while the fatality caused by cardiovascular disease has decreased by only 12.54%.The premature mortality of stroke accounted for 72.64%of the premature mortality of cerebrovascular diseases,while that of coronary heart disease accounted for 0.16%of the premature mortality of cardiovascular diseases.Conclusion1.Since 2010,the incidence of stroke and coronary heart disease in Suzhou has risen rapidly,but the fatality rate of stroke has declined gradually.Additionally,the coronary heart disease has risen slightly.The proportion of stroke deaths in cerebrovascular disease deaths gradually decreased,while the proportion of coronary heart disease deaths in cardiovascular disease deaths did not change significantly.2.Since 2008,the premature mortality of stroke has a significant impact on the probability of premature death of cerebrovascular diseases,while the premature mortality of coronary heart disease has no significant impact on the probability of premature mortality of cardiovascular diseases.3.Since 2000,the life expectancy in Suzhou increases continuously.Moreover,its trend is related to the probability of premature death of cerebrovascular diseases.Part 2:Study on the prevention and control for cardiovascular and cerebrovascular diseases in Suzhou.ObjectliveTo evaluate the effectiveness and existing problems of prevention and control measures for cardiovascular and cerebrovascular diseases in Suzhou,and to analyze the weight of various prevention and control measures on the premise of effective prevention and control.Method1.The death data of relevant years including the death data of cardiovascular and cerebrovascular diseases in Suzhou were collected and collated between 2010 and 2014.The comparative regions were divided into the demonstration areas and the non-demonstration areas of comprehensive prevention and control of chronic diseases.2.A special survey was used to obtain the evaluation information of prevention and control measures of cardiovascular and cerebrovascular diseases in Suzhou from 4 groups,including community medical workers,disease control professionals,health administrators and chronic disease experts,and to evaluate the weight of various prevention and control measures by analytic hierarchy process.3.The database was built and statistically analyzed with SPSS software(21.0),including correlation analysis,parameter test(t test)and non-parameter test(χ2 test).The weight values of various measures were calculated by using the multiple comparison,consistency test and related formulas introduced by the analytic hierarchy process.The weight value was calculated by compiling the formula in Excel.Results1.The awareness rate of six risk factors of hypertension and diabetes in demonstration area was 43.04%of high risk object standard,50.50%of nutrition label,86.55%of self-waist,96.67%of self-weight,86.12%of blood pressure and 57.09%of blood sugar,respectively.All of which were significantly higher than those in non-demonstration area,as 20.22%of high risk object standard,42.18%of nutrition label,57.71%of self-waist,89.21%of self-weight,55.20%of blood pressure and 18.92%of blood sugar,respectively.2.The index of the premature mortality of cerebrovascular diseases has reduced 0.51 from initial stage of evaluation to late in the demonstration area.It is significantly higher than those in non-demonstration area(0.26,P=0.037).However,the index of the premature mortality of cardiovascular diseases has risen 0.08 in demonstration and reduced 0.18 in non-demonstration(P=0.157).3.The premature mortality was significantly associated with the life expectancy in demonstration area(r=-0.830,P=0.003).However,no significant relationship was detected between the premature mortality and the life expectancy in non-demonstration(P=0.365).4.The weight of prevention measures are 24.94%of health life style,24.08%of heath education,the 18.27%of screening and management for high-risk groups,16.77%of control smoke and 15.95%of management diseases,respectively.Conclusion1.The awareness rate,medication rate and control rate of hypertension and diabetes mellitus patients in Suzhou city gradually increased within 15 years.The awareness rate of behavioral nsk factors in demonstration area was significantly higher than that in non-demonstration area.However,the premature mortality of cerebrovascular disease in Suzhou demonstration area was significantly higher than that in non-demonstration area.There was no significant diffierence in the premature mortality of cardiovascular disease between demonstration area and non-demonstration area.2.Among the five effective comprehensive prevention and control measures,the national health lifestyle action and health education have the greatest weight compared with the screening and management of high-risk groups,tobacco control and disease management.
