| Objective:To analyze the characteristics and imaging features of pulmonary infection in patients with type 2 diabetes mellitus with pulmonary infection.Methods:A retrospective study was conducted in 120 patients with type 2 diabetes mellitus complicated with pulmonary infection who were hospitalized in our hospital from April 2016 to January 2018.All patients underwent CT thin layer examination.The thickness interval is 2.0mm and The layer is 1.0mm.Through sputum bacterial culture,bronchoalveolar lavage tissue bacterial culture or blood bacterial culture,analysis of bacterial infection,bacterial infection types and attributes,blood sugar control and disease outcomes.Results:①The detection rate of pathogenic bacteria in 120 patients was 82.5%(99/120).The main pathogenic bacteria were Streptococcus pneumoniae,Klebsiella pneumoniae,Staphylococcus aureus and Pseudomonas aeruginosa.There were 85 cases infected with single bacteria,and 14 cases were infected with two species of bacteria.② Among the 120 patients,of which 54 had lobar pneumonia on CT scan,showing consolidation with segmental or lobular distribution of lesions.Bronchial gas phase could be seen in consolidation.The boundary near the pleura was clearer,the remaining boundary was unclear and less smooth.The volume of consolidated lobes was usually the same as that of normal lobes.24 cases of lobular pneumonia,mainly manifested as patchy shadows along the bronchus and blood vessels,some lesions with lobular emphysema or atelectasis.20 cases had pulmonary abscess,mostly located in the lower lobes of both lungs.CT examination showed that the lesion was accompanied by one or more air-liquid flat cavities.In some cases,the cavity wall was obviously enhanced after enhancement.There were 7 cases of fungal infection,including 5 cases of pulmonary aspergillosis.CT showed round soft tissue nodules in the cavity,and some nodules could roll in the cavity.The other 2 cases were pulmonary candidiasis.CT showed a large number of solid lesions,with multiple liquid holes inside.Fifteen cases of pulmonary tuberculosis were active pulmonary tuberculosis.CT examination showed mainly cavitation,proliferative nodules and exudative consolidation.Of the 15 patients,19 segments developed lesions,including 11 in the posterior and dorsal lobes,8 in the infrequent regions(basal,anterior and anterior middle lobes),and all had hilar mediastinal lymphadenopathy.③The number of bacterial infections was not related to the prognosis of patients(p<0.05).Of the 120 patients,95 had good blood glucose control and 25 had poor control.The cure rate of patients with better blood glucose control was significantly higher than those with poor control,and the rate of ineffectiveness was low,the difference was statistically significant(p<0.05).Conclusions:1.The patients with type 2 diabetes are easily associated with pulmonary infection,and CT examination can better reflect the lung disease.2.Active control of blood sugar can change the cure rate of the disease and improve the prognosis of the disease. |
