The Value Of Neutrophil Gelatinase Associated Lipocalin In The Early Diagnosis Of Renal Injury Of Type 2 Diabetes

Objective Investigate the changes and significance of NGAL in blood and urine at various stages of type 2 diabetic nephropathy progression.Look for the similarities and differences between NGAL and conventional indicators.Explore the value of NGAL for early onset detection and damage monitoring of type 2 diabetic nephropathy.Methods A total of 105 patients with T2DM include 20 early onset T2DM patients who were admitted to the Department of endocrinology of Suzhou Municipal Hospital in July 2016-February 2017 were selected,with 30 people who do physical examination at the same time and had normal level in blood sugar,blood lipids,liver and kidney function.The two groups had the same gender composition.The subjects were collected fresh morning urine 10ml,2000r/min centrifugal 5min,the supernatant was stored in the-80? low temperature refrigerator;collect blood fasting venous blood samples,separate serum stored in-80? low temperature refrigerator,determination of blood and urine NGAL,Cys C,alpha 1MU,urine 2MU,serum high-sensitivity C reactive protein(hs-CRP)glucose and lipid related indicators.SPSS 17.0 statistical software is used for data analysis and processing.Results I?Compared with group of T2DM,the group of early-onset T2DM has many differences in blood glucose?blood lipid and inflammation index(p<0.05).2?Compared with the control,the group of normal urine with T2DM has many differences in uNGAL?sCys uCys C and ? 2MU index(p<0.05).And there are obvious differences in urinary NGAL between normal albuminuria group and normal early-onset T2DM group(p<0.05).3?There is a correlation among uNGAL and other index,sNGAL?sCys C?uCys C?? 2MU?GA and hs-CRP,in group of T2DM.The combination of uNGAL and sNGAL for diagnosis of DN are better than other routine index(sensitivity 0.85 and specificity 0.85).Conclusion The severity of kidney damage of early-onset T2DM is more serious than normal T2DM.And there is a vaster foreground in detection of uNGAL for diagnosis of early-onset T2DM.The joint detection of uNGAL and sNGAL have a great prospect in early diagnosis of type 2 diabetic nephropathy.
